- The purpose of this FBO posting is for informational purposes. The Department of Veterans Affairs, Network Contracting Office 10, VA Healthcare System has determined that it is in the best interest of the government to award to EOI, Inc. DBA Medical Resources. They are located at 8377 C Green Meadow Ste C, Lewis Center, OH 43035. This is a sole source procurement per there only being one source per FAR 8.405-6 (a) (1)) (i) (B); The contractor has proprietary rights being the original manufacturer (OEM) of the Checkpoint System. Only the original equipment manufacturer can upgrade the existing system utilizing the existing equipment, components, and infrastructure. The items are available on FSS Schedule GS-07F-0031T. EOI is the exclusive source for the Support and Maintenance Services for the CheckPoint System. There is no other source of these Services available. Manufacturer does not have any authorized vendors or field technicians to provide competition. Services will be performed to provide checkpoint system annual maintenance and support to include: 1) Temp 150 (based on number of hardware components) 2) Temp 140 recalibration for laboratory external sensors 3) Temp 141 recalibration for laboratory temp humidity sensors 4) TEMPTRAIN recalibration labor services for main facility including travel time and expenses with CBOC (Community Based Outpatient Clinic) via UPS. (based on per piece of hardware component. Award will be processed to cover timespan date of award from base year 10/01/18 09/30/19. There are four one- year options available as follows: option year one (10/01/19 09/30/20), option year two (10/01/20 09/30/21), option year three (10/01/21 09/30/22, option year four (10/01/22 09/30/23). Per FAR 5.207 C 16 II All responsible sources may submit a capability statement, proposal, or quotation, which shall be considered by the agency.
