Addendums to FAR 52.212-1 INSTRUCTIONS TO OFFERORS--COMMERCIAL ITEMS and FAR 52.212-2 -- Evaluation -- Commercial Items. 1.) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. 2.) Solicitation number W911RQ-19-Q-0002 is being issued as a request for quotation (RFQ). 3.) This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-100. 4.) This RFQ is a 100% small business set-aside for NAICS 811310 with a small business size standard of $7.5. 5.) Description of requirements: Red River Army Depot (RRAD), Texarkana, Texas, 75507, has a service requirement for a preventive maintenance (PM) service contract on eight Fume Hoods at Red River Army Depot: six (6) Duralab Fume Hoods, one (1) Labconco Fume Hood, one (1) St. Charles Radiation Fume Hood. SEE SOW ATTACHMENT FOR DETAILS. 7.) Delivery Information for date(s) and place(s) of delivery and acceptance and FOB point: Period of Performance (PoP): The PoP shall be one (1) Base Year of 12 months and four (4) 12-month option years. Preventive maintenance visits shall be two (2) times during the contract year, space evenly apart. SEE SOW ATTACHMENT FOR DETAILS 8.) The provision at 52.212-1, Instructions to Offerors -- Commercial, applies to this acquisition and there is an addenda to this provision. 9.) The provision at 52.212-2, Evaluation -- Commercial Items, applies to this acquisition and there is an addenda to the provision. 10.) A completed copy of provision 52.212-3 Alt 1, Offeror Representations and Certifications -- Commercial Items, shall be included with your quote. 11.) A completed copy of provision 52.219-3 Alt 1, Small Business Program Representations, shall be included with your quote. 12.) The clause at 52.212-4, Contract Terms and Conditions -- Commercial Items, applies to this acquisition and there are no addendum to the provision. 13.) The clause at 52.212-5 (DEV), Contract Terms and Conditions Required To Implement Statutes Or Executive Orders -- Commercial Items (Deviation 2018-O0021), applies to this acquisition. 14.) The date, time and place offers are due: 05 November 2018 at 12:00 pm central time via FBO.GOV. 15.)
