Scope of Work This notice is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The Navajo Area Indian Health Services, Chinle Comprehensive Health Care Facility (CCHCF) is requesting for submittal of offers for a contractor to provide Preventive Maintenance Services for our Carl Zeiss Equipments at three (3) of our locations: Chinle Comprehensive Health Care Facility in Chinle, Arizona, Pinon Health Center, Pinon, Arizona, Tsaile Health Center, Tsail, Arizona. This combined synopsis/solicitation is a Indian Small Business Economic Enterprises set aside. The period of performance will be from 10/01/2018 - 09/30/2019. CCHCF is based in Chinle, Arizona (Northeast Arizona near Canyon De Chelly National Monument). The CCHCF is a 60 bed hospital which serves as the health care hub for the region. The medical staff includes Family Physicians, Internists, Pediatricians, General Surgeons, OB/GYN's, Anesthesiologists, and a Psychiatrist. In addition to routine outpatient and inpatient primary care, services available to our patients include: Adult Intensive Care, General Surgery (including laparoscopic surgery), routine and operative Obstetrics, and 24-Hour Emergency Room Services. Health care services are provided to approximately 37,000 active users. EVALUATION CRITERIA: CCHCF reserves the right to issue a single award or multiple contracts to any contractor(s) whose quote represents the best value as define by FAR 2.101. In determining best value, Price and other Evaluation Factors will be considered: Past Performance, and Experience. The Evaluation Factors and significant sub factors when combined are significantly more important than cost or price. The socio-economic status of an offer may also be considered, should one or more quotes represent the best value. Multiple awards may be considered.
