- Attachment 3 - Dept of Labor - Wage and Hour - Augusta and Brunswick - 2015-4009 Attachment 5 - FAR and DFARS Clauses and Provisions Attachment 2 - Dept of Labor - Wage and Hour - Bangor - 2015-4001 Attachment 4 - Contract Line Item Pricing Sheet and Delivery Dates Attachment 1 - Performance Work Statement Combined Synopsis/Solicitation for Medical Readiness in Support of Individual Medical Readiness (IMR) and Fully Medical Ready (FMR) for Periodic Health Assessments (PHA) on eligible Service Members. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6 as supplemented with additional information included in this notice. This announcement constitute the only solicitation offerors are being requested and a written solicitation will not be issued. This Request for Quote (RFQ) is being issued in accordance with (IAW) FAR 12.203 - Procedures for Solicitation, Evaluation, and Award and pursuant to FAR 13.5 - Simplified Procedures for Certain Commercial Items. Each proposal will be evaluated in accordance with FAR 13.106-2 -Evaluation of Offers under Simplified Acquisition Procedures and FAR 12.6- Streamlined Procedures for Evaluation for Commercial Items. The solicitation number is W912JD-19-Q-0001. Synopsis: A requirement for to conduct and complete the various components of IMR/FMR, Medical and Dental Readiness on all eligible Service Members (SM) of the Maine Army National Guard (MEARNG.) This notice incorporates provisions and clauses in effect through the Federal Acquisition Circular (FAC) 2005-100 effect August 22, 2018 and the Defense Federal Acquisition Regulation Supplement (DFARS). The complete text of any of the clauses and provisions may be accessed in full text at http://farsite.hill.af.mil/ or http://farsite.hill.af.mil/vmdfara.htm This is a Total Small Business Set-Aside. The associate North American Industry Classification System (NAICS) Code for this procurement is 621399 (Offices of all other miscellaneous health practitioners) and the small business set-aside standard is $7.5 Million. The Product Service Code is Q403 (Evaluation & Screening). Deadlines: Questions concerning this solicitation or requests for classification shall be made in writing via email to Darlene Howes at darlene.r.howes.civ@mail.mil by Monday, 5 November 2018. Any question received after Monday, November 5th will not be answered. Questions will only be accepted via email and will be posted to the FBO.gov web-site. Quote packages are due by 4:00 PM Eastern Local Time on Thursday, 8 November 2018. Quotes shall be submitted via email to darlene.r.howes.civ@mail.mil. In the subject line: W912JD-19-Q-0001. Quotes shall not be accepted by any other means. All quotes received after this due date and time shall be considered late in accordance with FAR 52.212-1(f). Offerors will receive an email confirmation that they quote was received within two (2) business days. Quote packages shall include a cover sheet that provides the following information: •Company Name •Point of Contact, to include email and phone number •Federal Tax ID Number •CAGE Code •Statement that includes acknowledgement of latest amendment number (if applicable). Failure to acknowledge the most recent amendment number, may render an offeror's quote non-responsive. Offers will be evaluated on the basis of information furnished with the Request for Quote (RFQ). Failure to submit the required information will preclude consideration of the offer for this requirement. See Attachment 5 - FAR Clauses and Provisions - IAW 52.212-2 - Evaluation - Commercial Items for additional details. Quotes shall be good for sixty (60) calendar days from closing date. Offeror must be certified in www.SAM.gov database prior to award, during performance and through final payment of any contract resulting from this solicitation. This will be a determining factor for award during the evaluation process of the offeror quote, if the SAM.gov certification is not current. https://www.sam.gov/portal/public/SAM/. If the offeror does not become registered in System for Award Management (SAM) database prior to the solicitation closing date, the Contracting Officer will proceed to award to the next otherwise successful registered Offeror. All information on the solicitation must be filled out completely and returned. Incomplete submissions will be determined not in conformance to the Solicitation and may be rejected. Please be sure to read the entire solicitation for sections that require vendor information to be submitted. The Government reserves the right to award one Firm Fixed Price contract resulting from this solicitation to responsible offeror. Any award resulting from this solicitation will be issued on a Standard Form (SF) 1449. FAR 13.101. The offeror has completed the annual all Representations and Certifications electronically found in the provisions within their profile in the System for Award Management (SAM) at http://www.sam.gov site. The description of all of the requirements are in the Performance Work Statement (PWS) hereby enclosed. (RFQ Attachment 1). Offeror shall use the attached Wage Determination WD 2015-4001 (Rev.-7) dated 07/17/2018 (RFQ Attachment 2) and WD 2015-4009 (Rev.-7) dated 07/17/2018 (RFQ Attachment 3) which is posted on www.wdol.gov for the location identified in the PWS. All proposals submitted in response to this solicitation shall reflect the use of current, updated fringe benefit rates. The Offeror's wage rate shall comply with the Department of Labor (DOL) wage rate minimum requirements for the labor category and Wage Determination provided by the Department of Labor - Wage and Hour - hereby enclosed as attachment two and three. Notice to the Offerors: Funds are not presently available for this acquisition. No contract award will be made until appropriated funds are made available. The Government reserves the right to cancel this Request for Quote, either before or after the closing date for receipt of proposals. In the event the Government cancels this announcement, the Government has no obligation to reimburse an Offeror for any costs. Discussions: The Government intends to evaluate quotes and make award without discussions. During the evaluation process, the Government may, at its discretion enter into clarifications or communications as needed. The Government need not enter into communications or clarifications with all Offerors. Contract Line Items Numbers (CLINs): Pricing Sheet (Attachment 4) Invoice: Contractor shall consolidate all medical services performed on an individual event basis and submit an invoice through Procurement Integrated Enterprise Environment (PIEE) - iRAPT within 10 business days post event.
