- 1. This is a Combined Synopsis/Solicitation for Commercial Items prepared in accordance with the format in FAR Subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation that will be issued; quotes are being requested and a written solicitation will not be issued. The Government anticipates a Firm-Fixed Price Purchase Order will be issued from this solicitation. 2. This requirement is being solicited as Request for Quotation (RFQ). The RFQ number is NIH-RFQ-5153620 3. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-101, dated October 26, 2018. 4. This requirement is solicited as a full and open competition. The associated North American Industrial Classification System (NAICS) code for this procurement is 332510. Its associated Small Business Size Standard is 750 employees. 5. List of Item(s): The vendor shall provide the following item in the quantities specified below. a. Anti-Ligature Conversion Kit for Schlage Mortise Lock. Part# 09-138-626 is specific for Schlage---Quantity---40 each. 6. Quotation Requirements: Quotation must reference: RFQ No. NIH-RFQ-5153620. Vendor's delivery time frame must be specified in their quote. Vendor must quote on the line item listed in this notice to be considered. The item must be quoted in the same quantities as listed in this notice. The quote must provide both the single quantity item and total quantity item pricing. Quotations must include any applicable shipping costs as a separate line item. 7. Delivery and Acceptance Deliveries required by the contractor shall be made F.O.B. destination to the address/addressee listed below: Delivery Address/Addressee: National Institutes of Health 9000 Rockville Pike Building 13, Platform C Bethesda, Maryland 20892 Unless otherwise specified, deliveries shall be made to the Delivery Point specified above, Monday through Friday (excluding Federal Holidays, between the hours of 8:30 a.m. and 4:30 p.m. Supplies or services scheduled for delivery on a Federal holiday shall be made the next business day. 52.252-1 Solicitation Provisions Incorporated by Reference (Feb 1998) This RFQ incorporates one or more solicitation provisions by reference, with the same force and effect as if they were given in full text. Upon request, the Contracting Officer will make their full text available. Also, the full text of a solicitation provision may be accessed electronically at this address: www.acquisition.gov/far 8. Provision 52.212-1 Instructions to Offerors - Commercial Items (OCT 2018) is incorporated by reference. 9. Provision 52.212-2 Evaluation - Commercial Items (OCT 2014) is incorporated by reference. The provision 52.212-2 incorporated above is completed as follows. Quotes received in response to this RFQ will be evaluated using the lowest-priced-technically-acceptable (LPTA) methodology. To be determined "Technically Acceptable" the offeror's proposed anti-ligature conversion kit must be equivalent to the specifications of the item listed. Award will be made to the vendor whose proposed item meets all specifications, whose total price is the lowest quote, and who provides the most desirable delivery time. The Government desires the shortest delivery time and delivery times may be considered in determining the purchase order award. 10. Provision 52.212-3, Offeror Representations and Certifications - Commercial Items (OCT 2018) applies to this acquisition. 11. Clause at 52.212-4, Contract Terms and Conditions-Commercial Items (OCT 2018), applies to this acquisition and no addenda to the clause are included. 12. Clause at 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders-Commercial Items (OCT 2018), applies to this acquisition. 13.
