Contract Title DSS Consult Tracking Manager (CTM) 3. Background One of the functions of a Veterans Administration Medical Center is to disposition and manage consults for many specialty areas. Over time, some of these consults age past compliance dates. When that occurs, minimum scheduling efforts must occur (such as phone calls and mailed letters to patients). The management of these consults is often done manually by paper, which results in extensive rework on a service-by-service basis. This manual method required an automated solution, which was addressed by DSS, Inc. Consult management software is a commonly provided service, if not a community standard, for most healthcare organizations today. Focus is on effective communication between specialists and the patients requiring specialty care. As the VA continues to manage many patients throughout the continuum of care, it becomes crucial to adopt real-time methods which will help the VA know the status of patients in scheduling specialty care service. This real-time software will eliminate extensive rework by support staff, thus realizing gains in efficiencies of support staff. 4. Scope The current process of reviewing open consults completeness and closure was completed via a manual process, and the results of the completed reviews were aggregated on a recurring basis. Utilizing the CTM software, it is expected that consult management time will be reduced by one hour per scheduler per day, and eliminate some of the human error variables in the review process. With the purchase of the aforementioned CTM software and consulting services, we will be able to: Provide a department/service view of consults, rather than patient-by-patient. Display and updates consult information in real time. Integrate with other VistA packages to provide up-to-date information on the same page. Create workflow efficiency by eliminating paper or duplicative systems. Ensure every team member has up to date work list for consults they need to process. Improve patient satisfaction by providing tools to help ensure timely completion of consults. Monitor user performance and view numerous metrics pertaining to consult aging and bottlenecks. Based on testing feedback from other VA facilities, usability of the software by the end user is one of its greatest characteristics. The CTM software offers: Dashboard views provides a complete snapshot of consult status for one or more departments. Real time analytics show status of all consults for more accurate tracking and reporting. Last comment entered is automatically displayed for easy viewing by user. Status of all open consults, but can also show information on cancelled and continued consults if desired. Detailed reporting and metrics that capture elapsed time consults remain open. Consolidates management, scheduling, patient information, view of actual consult, and all other pertinent details into single screen. Enables visibility of full patient record while viewing consult. Standard canned text for comments on all actions. Productivity reports. Easy tracking of quantity of calls and letters to patients, including display of letters over 14 days. Filtering options allow users to narrow down consults that need attention for the users specific workflow. 5. Specific Tasks. Document Storage Systems, Inc. s (DSS) Consult Tracking Manager is a web-based solution that streamlines workflows, provides easy administrative oversight, and helps to ensure patients are called and scheduled in a timely manner. It improves overall outcomes for administrators and consult managers, providers and patients alike. Consult Tracking Manager is a VistA-integrated solution that provides real-time, up-to-date dashboard views of the status of all consults, displayed by service line. Users can take necessary actions on the consult from within the dashboard without going to CPRS; all data is maintained in VistA in the appropriate files. Standard comments are available within Consult Tracking Manager, and specific Call and Letter actions allow the application to track productivity and alert users when calls or letters are due, rather than requiring a review of the consult activity log within CPRS. Users can view linked appointments or create links when scheduling clerks inadvertently miss that step. Quantity: 1 perpetual license 7. Performance Monitoring Contractor work will be evaluated upon completion by the Clinic Practice Management Service, Albany NY, to ensure adequate and proper installation and work is completed to specified code. 8. Security Requirements Contractor will not have any access to patient information and data. VA security policies do not apply. 9. Government-Furnished Equipment (GFE)/Government-Furnished Information (GFI). None. 10. Other Pertinent Information or Special Considerations. None. 11. Risk Control This is a firm fixed price contract. SEWP vendors have a long standing relationship providing equipment, parts and services to the VA. Risk to the Government is considered minimal. 12.
