Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. This solicitation RFQ-36C25019Q0177 is issued as a request for quote (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-94, 2005-95/ 01-19-2017. The North American Industry Classification System (NAICS) number is 339113 and the business size standard is 750 employees. BRAND NAME ONLY 1 EACH Certus 140 Certus 140- 2.4GHz Ablation System System Features The system has three (3) independent 2.45 GHz microwave generators, each capable of producing up to 140W each. One, easy to use, touch-screen user interface controls the system. Up to 3 microwave ablation probes can be connected to and powered by the system at one time. The maximum selectable power for the system is 140W when one probe is connected, 95Wper probe when 2 probes are connected and 65W per probe when 3 probes are connected. An intermediate junction box or Power Distribution Module (PDMV) reduces system set up complexity. A CO, based cooling system ensures the non-active portion of the probe does not exceed temperature requirements. Additionally, the CO2 enables the Tissu-Loc function, which can be used to adhere or stick the probe in place prior to starting ablation therapy. The system uses two (2) F-sized CO, cylinders. When a tank in use empties, the system will automatically switch to using the other tank and notify the user to replace the empty tank Services provided by the vendor The vendor will ship the machine to the VA Ann Arbor Healthcare System, 2215 Fuller Rd, Ann Arbor, MI 48105 (Surgery Service). A local NeuWave Technical Service Specialist shall come to the hospital to install and perform pre-use testing. NeuWave shall assist with un-crating, set up, and installation. NeuWave shall provide clinical support/in-services for the staff until they are comfortable with the system. The proposed contract listed here is Set-aside 100% for SDVOSB. FOB destination. Deliver to 2215 Fuller Road, Ann Arbor, MI 48105. Offerors must comply with all instructions contained in provision 52.212-1, Instructions to Offerors-Commercial, applies to this acquisition. 52.204-7; 52.204-16; 52.212-2, Evaluation - Commercial Items: FAR provision 52.212-2 does not apply to this solicitation. Award will be based on lowest price technically acceptable offer. 52.212-3, Offeror Representations and Certifications-Commercial Items, a completed copy of this provision shall be submitted with the offer unless SAM registered which must be noted in the quotation. 52.212-4, Contract Terms and Conditions-Commercial Items.
