- This Sources Sought Notice is not a formal request for proposals or quotes. No formal solicitation document exists at this time. The Dental Clinic within the Durham Veteran Affairs Medical Center (VAMC), 508 Fulton St, Durham, NC 27705, has a requirement for dental laboratory services. The contractor shall fabricate various fixed prosthesis on natural tooth abutments and/or implant abutments to restore the form, function, and aesthetics of Veterans Affairs (VA) patients. The contractor shall perform twice daily pickup and delivery of lab casework at the Durham VA Dental Clinic site. Physical location of dental lab site must be local proximity to the dentist office to provide same day services. See below for additional information on the projected dental laboratory services. Complete set of dentures to include model, custom tray, bite block articulation, set up reset process& finish. Cast Partial Denture with two wrought wire clasps to include. include model, custom tray, bite block, articulation, set up reset. Cast Partial Denture with Two Flex Clasps to include (include model, custom tray, bite block, articulation, set up, reset, process & finish). Acrylic Partial Denture with 2 Wrought Wire Clasps to include (include model, custom tray, bite block, articulation, set up, reset, process & finish). Various Repairs to include (repair partial denture any type, repair complete denture any type, rebase complete/partial dentures, add teeth, repair cast high noble and porcelain crowns). Crown /Bridge/Onlays - full gold crowns and onlays (include model, articulation, cast, process & finish). Crown/Bridge Onlay Porcelain Fused to Gold (include model, articulation, cast, process & finish). CAD-CAM Crowns (incl model, set-up, articulation, post-surgical prosthodontic guides). CAD-CAM Implants (incl model, articulartion, surgical stent). Cast Implant Abutments. Implant supported prosthesis (Non-CAD-CAM). Core build ups with post (cast and pre fabricated). Stents, carrier trays, occlusal guards. The potential requirement may be for a base period of 12 month plus four additional 12-month option periods. The applicable NAICS Code is 339116 (Dental Laboratory) with a size standard of 500 employees. This is a SOURCES SOUGHT announcement only. The purpose of this notice is to conduct market research and obtain information from qualified businesses. This is not a request for bids, quotes, or proposals. All responsible sources must submit a capability statement addressing its capabilities, capacity, and location which shall be considered by the agency. This notice shall not be construed as a commitment by the Government to ultimately award a contract, nor does it restrict the Government to a particular acquisition approach. All information submitted in response to this announcement is voluntary; the Government will not pay for information requested nor will it compensate any respondent for any cost incurred in developing information provided to the Government. Interested parties submitting a response to this inquiry must include the following information: Capability statement that demonstrates the firm s ability to meet the above requirement. Contractor laboratory locations that will support the requirement. Name and address of company; NAICS Code; DUNS number: business size (large, small, VOSB, or SDVOSB); as well as any applicable socioeconomic categories. It is the potential vendor s responsibility to monitor the FEDBIZOPPS website at https://www.fbo.gov for release of any future amendments or solicitation that may result from the sources sought.
