Contractor shall maintain or restore the listed equipment to operating specifications according the manufacturer's recommendations. Preventive maintenance must include, but is not limited to, the following: Examination, alignment, lubrication, calibration and verification of proper operating conditions of the equipment listed. All services for this contract must meet manufacturer's performance and technical specifications. Repairs will consist of the replacement of unserviceable or defective components other than cables and safety discs. Hard drives or memory devices will not be returned to the contractor if the equipment fails. Labor and materials in support of repairs must be charged at contractor's applicable rates for any of the following conditions. Such charges are over- and- above the firm fixed prices for preventive maintenance: Malfunction or damage due to improper use of equipment as outlines in the manufacturer's literature. Malfunction or damage due to unauthorized modification or repair of the system by the customer. Malfunction or damage due to unauthorized equipment interfaces. Any malfunction or damage due to accident, misuse, or neglect. The Contractor must furnish all parts as necessary to maintain the equipment covered by this contract. Services will be performed at the South Texas Veterans Health Care System, Audie L. Murphy VA Hospital, 7400 Merton Minter St., San Antonio, Texas 78229. The NAICS Code for this procurement is 811219 Other Electronic and Precision Equipment Repair and Maintenance with has a Small Business Size Standard of $20.5M. This action is accomplished under FAR 13.106-1(b)(1)(i), Soliciting from a Single-Source. This notice is not to be considered a request for quotations or proposals. No contract will be awarded based on offers received in response to this notice; however, any offers received may be used to determine if a comparable source is available and more advantageous to the government. All responsible sources may submit a response; which, if timely received, will be considered by the agency. To avoid Gray Market purchases, any response received from other than the OEM must contain documentation showing OEM training certification on the equipment listed and a letter from the OEM stating the company is an authorized distributor and/or service provider for the equipment listed. If parties choose to respond, any cost associated with preparation and submission of data or cost incurred in response to this announcement shall be the sole responsibility of the contractor and will not be reimbursed by the Government. A determination by the Government not to compete this proposed requirement based upon responses to this notice is solely within the discretion of the Government. If no affirmative written response is received by the response date and time, a purchase order will be issued to Terumo Cardiovascular Group without any further notice.
