Description
This is a Sources Sought notice. This is NOT a solicitation for proposals, proposal abstracts, or quotations. The purpose of this notice is to obtain information regarding the availability and capability of all qualified sources to perform a potential requirement. Background: The Louisville Robley Rex VA Medical Center, located at 800 Zorn Avenue Louisville, KY 40206, has a requirement for Neurology On-Call Services as described in the attached PWS. Purpose and Objectives: Your responses to the information requested will assist the Government in determining the appropriate acquisition method. NAICS code: 621999 and sized standard is $15 million employees. NOTE: Respondent claiming SDVOSB and VOSB status must be registered and CVE verified in VetBiz Registry https://www.va.gov/osdbu/. Project requirements: The contractor shall provide complete neurology on-call services to the Louisville VA Medical Center beginning 1 March 2019 through 28 February 2020 with four (4) one-year option periods. The contractor will include a minimum of two key personnel to be credentialed and available for scheduling to meet the requirements of the contract. The Contractor shall provide Board Certified /Board Eligible Neurology Physician Services on-site for General Neurology on-call, in accordance with the specifications contained herein to beneficiaries of the Department of Veterans Affairs (VA) and the Robley Rex VA Medical Center in Louisville, Kentucky. This contract does not include a need for Stroke service to be included. The Contractor s qualified Neurology physician(s) shall provide 24 hours of on-call services, Thursday Sunday, each week (4 days a week), plus government observed holidays and periodic staff vacation days. Except in cases of emergency, requirements for on-call coverage will be scheduled in advance. Coverage will include consultation with the requesting Robley Rex VAMC s physicians regarding the appropriate medical care of veterans presenting with neurological conditions and inpatient follow-up visits for any unstable inpatient. Initial response is expected within 30 minutes. Call will begin at 8:00am Thursday, and will end at 8:00am Monday. Anticipated period of performance: 1 March 2019 through 28 February 2020 for with four (4) one-year option periods. Capability statement /information sought: Response to this announcement shall not to exceed 10 pages and should include the following information: (1) A tailored capability statement indicating the firm s ability to provide a product and related services; (2) The respondents DUNS number, organization name, address, point of contact, and size and type of business (e.g., SDVOSB/VOSB, etc,) pursuant to the applicable NAICS code; (3) Associated contract vehicles (i.e. GSA/FSS; SEWP); (4) Information should be provided electronically in a Microsoft Word or Adobe PDF format.
