- VETERANS AFFAIRS MEDICAL CENTER, PHILADELPHIA, PA STATEMENT OF WORK Veterans Health Affairs, Network Contracting Office 4, is issuing this request for quote in order to identify capable firms to provide model number TH100-UG Image1 S H3-Z HIGH DEF CAMERA HEAD, FOR H3-LINK MODULE units compatible with Karl Storz Image 1 cameras (QTY 4) and model number 39301HCTS-ZBZ STERRAD COMPATIBLE HIGH DEFINITION CAMERA TRAY, 15.48 X 10.10 X 2.41 (QTY 4) for the Philadelphia, PA VA Medical Center. The units must be rated for commercial use to withstand heavy use and be within hospital specification standards. This solicitation is brand name or equal. This solicitation is a complete Small Business Set Aside. Responses must be submitted by January 10th, 2019 by 9:00AM Eastern Standard Time (EST). Submit responses to the information requested above via email to rashaun.thomas@va.gov. All SDVOSB and VOSB firms that respond shall include proof of CVE certification via www.vip.vetbiz.gov. All small business firms that respond shall include proof of small business status via their Representations and Certifications in accordance with (FAR 4.1102 Policy). All firms that respond must be registered in the System for Award Management via https://sam.gov/SAM/. All firms who respond must submit their Duns & Bradstreet number via https://www.dnb.com/. All firms who respond must submit their tax ID number. While SDVOSB/VOSB contractors are preferred, all capable contractors are welcome to respond to this source sought notice for market research purposes, and shall include as part of their response a brief capability statement that covers the information in the following tentative statement of work. The results of this market research will assist in the development of (1) the requirement, and (2) the acquisition strategy (e.g., socioeconomic set-aside, full and open competition, etc.). VA assumes no responsibility for any costs incurred associated with the preparation of responses. All vendors shall use the price schedule located in section B to quote the price for each line item. Attachments are acceptable, each vendor MUST fill out the information requested in pricing schedule located in section B for each individual line item. Sample Statement of Work: Background: The equipment must be/have: These cameras are needed to support and be compatible with the existing Karl Storz flexible and rigid scopes already in use at the Corporal Michael J. Crescenz VA Medical Center. Performance Requirements: IMAGE1 S H3-Z compatible HIGH DEF CAMERA HEAD, for H3-LINK MODULE model number TH100-UG, brand name or equal. STERRAD compatible HIGH DEFINITION CAMERA TRAY, 15.48 X 10.10 X 2.41 model number 39301HCTS-ZBZ, brand name or equal Manuals: The Contractor shall provide one (1) copy of an owner s manual for each device. The contractor shall provide two (2) copies of the repair manuals to Biomedical Engineering for use by the designated section responsible for inspection, maintenance and repair. Warranty: The contractor shall provide at least a two (2) year manufacturer s warranty on all camera head parts and labor, and one (1) year manufacturer s warranty on the STERRARD sterilization trays. Delivery: Contractor shall deliver all equipment to the Corporal Michael J. Crescenz VA Medical Center located at 3900 Woodland Avenue, Philadelphia, PA. 19104. Delivery must be completed by January 31, 2019. Hours are Monday through Friday 7:30am to 3:30pm The Contractor shall coordinate all deliveries, staging areas, and parking arrangements with the VA Police and Warehouse. The Contractor shall remove all related shipping debris and clean up any construction associated with delivery of specified items. Contractor shall remove all packing from the Corporal Michael J. Crescenz VA Medical Center premises. The Contractor shall be responsible for any damage to the building that occurs due to Contractor error or neglect. SITE CONDITIONS: There shall be no smoking, eating, or drinking inside the hospital INSPECTION AND ACCEPTANCE: The warehouse shall receive all equipment prior to Biomedical Engineering inspecting the equipment. Disputes shall be resolved by the Contracting Officer DELIVERY/STORAGE REQUIREMENT: Deliver materials to job in manufacturers original sealed containers in good repair. Deliver specified items only when site is ready. Store equipment in dry conditions inside facility. Any government requested delayed delivery up to 90 days after initial delivery date, shall be at no additional cost to the Government DELIVERABLES: Operation and Maintenance Manuals 1 copy of the owner s manual for each device Maintenance Manuals two (2) copies of the repair manuals to Biomedical Engineering for use by the designated section responsible for inspection, maintenance and repair. Paper or electronic is acceptable. The vendor must provide maintenance services, or preventive maintenance training to Users and Biomedical Technicians when appropriate, to meet at a minimum Joint Commission standard OPERATOR TRAINING: Technical training must provide Biomedical Engineering with the tools and knowledge to fully operate and maintain the system. Scheduling of all training shall be coordinated with the Corporal Michael J.
