- SOURCES SOUGHT NOTICE This is not a request for quotes or proposals. No formal solicitation document exists at this time. Network Contracting Office 4 (NCO 4) is posting this Sources Sought Notice for the purpose of locating capable small businesses, including Service Disabled and Veteran Owned small businesses, to provide On-Site RADIATION PHYSICIST SERVICES (NON-PERSONAL SERVICE) for the Wilkes Barre VA Medical Center. Place of Performance: Services shall be provided on site, at the Wilkes Barre Veterans Affairs Medical Center (WBVAMC) located at 1111 East End Boulevard, Wilkes Barre, PA, the Allentown Outpatient Clinic (OPC) located at 3110 Hamilton Boulevard, Allentown, PA and the Sayre OPC located 1537 Elmira Street, Sayre, PA. The anticipated contract is to be awarded for a period of five years. The NAICS code is 541380 and the size standard is $15.0 million. A DRAFT Performance Work Statement (PWS) is attached hereto. The Contractor shall provide all resources necessary to accomplish the services described in the attached PWS. All professionals providing services under the terms of this contract will be appropriately certified and/or licensed as described in the PWS, and shall possess the requisite skills and experience to perform the services as required. Contractor s physician(s) shall be technically proficient in the skills necessary to fulfill the Government s requirements. Contractor shall not perform inherently governmental functions. This Sources Sought is an advance notice for information and planning purposes only and is not a solicitation. Interested organizations may submit their capabilities and qualifications to perform the effort in writing to the identified point of contact. Such capabilities/qualifications will be evaluated solely for the purpose of market research. Sources shall include in their information: 1. Name of Business with a capabilities statement stating their experience in providing the same or similar work described in this notice. 2. A point of contact with phone and e-mail information. 3. Business status (Socio-Economic Factors: Service-Disabled Veteran-Owned Business, Small Business, Veteran Owned Small Business), business size based on NAICS 541380. All Status are encouraged to respond to this notice. 4. If currently a vendor under the VA Federal Supply Schedule (FSS) please provide contract number(s) and listing of Special Item Number(s) (SIN) covered. Please include if you service this area. If not a vendor with a current FSS contract please so state in the capability statement. This notice is not a commercial solicitation and the Government will not pay for any information submitted, or for any costs associated with providing the information. Prospective Contractors are reminded that in accordance with FAR 4.11 they shall be registered in the System for Award Management (SAM) database and shall complete electronic annual representations and certifications in SAM prior to award of a contract. SAM is located at https://sam.gov/SAM/ and consolidated the capabilities of CCR/FedReg, ORCA, and EPLS. There is NO cost to use SAM.
