- VA SAN DIEGO HEALTHCARE CENTER TASK CHAIRS RFQ - 36C26219Q0235 This is a combined synopsis/solicitation for commercial supplies prepared in accordance with the format in Subpart 12.6, as supplemented with the additional information included in this notice as a set aside for service disabled veteran owned small businesses (SDVOSBs) and veteran owned small businesses (VOSBs). This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. This is a request for quotes under solicitation number: 36C26219Q0235 Responses are due by 12:00 pm (PST), January 24, 2019 Solicitation 36C26219Q0235 is issued as a Request for Quote (RFQ) and constitutes the only solicitation. The Government intends to award a single Firm-Fixed Price (FFP) award contract. The Department of Veterans Affairs SAN DIEGO Healthcare System has a requirement for LEAP CHAIRS for the Medical Center. The Contractor shall furnish all materials/service at the Veterans Affairs, 3350 La Jolla Village Drive, SAN DIEGO, CA 92161. The Government is seeking a vendor who can provide the following items for the SAN DIEGO VA Healthcare System: LINE 1 Leap Chair Upholstered - Soft Casters QUANTITY - 200 LINE 2 Leap Chair Upholstered - Hard Casters QUANTITY - 200 LINE 3 Steelcase Com Charges for Vinyl QUANTITY - 400 See salient characteristics document. This solicitation is for brand name (exact match or equal) and offerors shall be an authorized distributor by the manufacture and must be able to offer full manufacture warranty all at a fair and reasonable price offering best value to the Government. 400 Leap task chairs or equal to be drop-shipped to the VA San Diego, 3350 La Jolla Village Dr., San Diego, CA 92161. VA will request 16 separate shipments to be coordinated throughout the year. Submitting Quote: Offeror(s) shall submit their quote on company letterhead and shall include the completed pricing forms, along with name, address, telephone number of the offeror, firm s DUNS#, total price. Offerors shall have completed FAR Clause 52.212-3 Offerors Representations and Certifications - Commercial Items in Systems for Award Management SAM at www.sam.gov as required by FAR Clause 52.212-3 Offerors Representations and Certifications - Commercial Items.
