- THIS IS A SOURCES SOUGHT NOTICE (a) The Government does not intend to award a contract on the basis of this sources sought notice or to otherwise pay for the information solicited. (b) Although proposal, offeror, contractor, and offeror may be used in this sources sought notice, any response will be treated as information only. It shall not be used as a proposal. (c) Any information received from a contractor in response to this sources sought notice may be used in creating a solicitation. Any information received which is marked with a statement, such as proprietary or confidential, intended to restrict distribution will not be distributed outside of the Government, except as required by law. (d) This sources sought notice is issued for purpose of collecting information about the availability of authorized distributors to provide prepared single unit doses of radiopharmaceuticals and associated materials to San Francisco VA Nuclear Medicine at SFVAMC. Vendor to provide prepared single unit doses or multi dose vials at the discretion of the facility ordering. Multi dose vials must be prepared and shipped in the manner described by Med Rad and in the lead and glass containers compatible with the Med Rad Intego Administration system. Shipping documentation must be readable by NMIS barcode reader. Orders will be placed each day for delivery the next business day (Monday through Friday). Doses must be delivered at least 30 minutes prior to the dose calibration times. Due to the demands of the Medical Center patient care requirements, contractor shall provide routine delivery and pick up as many times as required by VAMCSF at no additional cost. For additional doses placed during normal business hours for same day delivery, orders must arrive within 2 hours of order placement. In the event requirements, may be determined in advance; a longer delivery time may be scheduled accordingly. Vendor must be licensed by the Nuclear Regulatory Commission and be regularly established in the business of providing radiopharmaceuticals. This is a sources sought notice only, no formal solicitation document exists at this time. Offerors response shall include the following information: - Reference Number 36C26119Q0275 - Provide copies of licenses and certifications by the Nuclear Regulatory Commission - Company Name, point of contact, title, phone number and email address - DUNS number - Type of business (Small or large) and socioeconomic status whether Service Disabled Veteran Owned, Veteran Owned, Hubzone, 8(a), Woman owned, Small Disadvantaged, etc. or VOSB company to provide supplies for blood culture analysis. - State if your company has any government contracts against which this solicitation could be ordered (e.g., GSA, SEWP, NAC, GSA BPA, etc.) or if your proposal is submitted as a open market quote. For any government contract, provide your contract number, terms and conditions; SIN the products fall under (if any), delivery schedule and contract expiration date. - Contractors shall identify the NAICS code for the product being offered as well as their size status. (e) Please email all inquiries to carol.lam@va.gov by January 24, 2019. Only emailed received directly from interested persons are acceptable. Receipt must be acknowledged on my desktop computer to be recognized as received. Authorized dealers: Experienced firms only who are authorized distributors, financially responsible and able to show evidence of the reliability, ability, experience, equipment, facilities and personnel directly employed or supervised by them to render prompt and satisfactory service. Offerors shall be registered in the System for Award Management (SAM) prior to award. Registration information is located at www.sam.gov.
