RFI# 36C26219Q0052_0001 Page 1 of 3 THIS IS AN AMENDMENT TO PREVISOUS REQUEST FOR INFORMATION (RFI) SOURCES SOUGHT IS ISSUED SOLELY FOR MARKET RESEARCH AND PLANNING PURPOSES ONLY AND DOES NOT CONSTITUTE A SOLICITATION. 1. Responses to this Sources Sought must be in writing. The purpose of this Sources Sought Announcement is for market research only to make appropriate acquisition decisions and to gain knowledge of Small Business Capability, including Service Disabled Veteran Owned and Veteran Owned Small Businesses (SDVOSB/VOSB) interested and capable of performing the work. 2. The NAICS for this requirement is 624229, Other Community Housing Services. 3. Interested and capable Contractors should respond to this notice no later than January 25, 2019 at 1:00pm PST by providing the following via email only to Flora.Ceponis@va.gov. (a) company name (b) address (c) point of contact (d) phone, fax, and email of primary point of contact (e) DUNS number (f) Type of small business, if applicable, (e.g. Services Disabled Veteran Owned Small Business(SDVOSB), Veteran-owned small business (VOSB), 8(a), HUB-Zone, Women Owned Small Business, Small disadvantaged business, or Small Business). (g) Section (g) has changed from: A capability statement that addresses the organization s qualifications and ability to perform as a contractor specifically for the work described in paragraph 5 below. Respondents are also encouraged to provide specific examples (e.g. contract number, point of contact information) of the Contractor s experience providing the same or similar supplies/services to that described in paragraph 5 below. Capability statement must specify the Service Provider Area that they are capable of providing services in and the number of beds available in such area. 4. SDVOSB/VOSB respondents. (a) If respondent is VOSB or SDVOSB, respondent is encouraged to provide proof of www.vetbiz.gov certification. In addition, SDVOSB/VOSB Contractors are asked to acknowledge that they understand the limitations on sub-contracting pursuant to FAR 52.219-14, Limitations on Sub-Contracting, which will be included in the impending solicitation. Further SDVOSB/VOSB contractors are asked to respond that they are capable of providing the requested services keeping within the parameters of this clause. (b) Small businesses, including SDVOSB/VOSBS are also advised that per 13 CFR, § 125.6 the prime contractor's limitations on subcontracting are as follows: General. In order to be awarded a full or partial small business set-aside contract with a value greater than $150,000, an 8(a) contract, an SDVO SBC contract, a HUBZone contract, a WOSB or EDWOSB contract pursuant to part 127 of this chapter, a small business concern must agree that: In the case of a contract for services (except construction), it will not pay more than 50% of the amount paid by the government to it to firms that are not similarly situated. Any work that a similarly situated subcontractor further subcontracts will count towards the 50% subcontract amount that cannot be exceeded. 5. Description: Description has changed from: The Department of Veterans Affairs (VA) Plan to End Homelessness Among Veterans calls for enhancing current homeless service capacity as well as developing new programs and initiatives in concert with community and federal partners. The intent of this contract is for the Veterans Affairs Greater Los Angeles VA Healthcare System (VAGLAHS) to engage a community provider to offer homeless Veterans services through a Safe Haven model of residential care. This model of HCHV Contract Residential Care will offer services to homeless Veterans enhanced by the services as defined in the Tasks and Deliverables. The VAGLAHS will contract for 80 beds (no more than 40 beds in one location) and associated services through procedures in line with their own business practices and contracting services. To the following: The Department of Veterans Affairs (VA) Plan to End Homelessness Among Veterans calls for enhancing current homeless service capacity as well as developing new programs and initiatives in concert with community and federal partners. The intent of this contract is for the Veterans Affairs Greater Los Angeles VA Healthcare System (VAGLAHS) to engage a community provider to offer homeless Veterans services through a Safe Haven model of residential care. This model of HCHV Contract Residential Care will offer services to homeless Veterans. The VAGLAHS will contract for a total of 80 beds in the Ventura Service Provider Areas (SPAs) 1 Antelope Valley, 2 San Fernando Valley, 3 San Gabriel Valley, and 5 West (with in LA County); no more than 10-40 beds in each SPA. Entities that are interested in providing Veterans these services should be aware that, although this contract seeks services for the VAGLAHS, HCHV is a national program serving Veterans throughout the country. The VAGLAHS is seeking a Contractor to assist with the provision of emergency residential placement and supportive services through the Health Care for Homeless Veterans (HCHV) program LDSH model. This contract will serve homeless, chronically homeless and/or vulnerable homeless Veterans with mental health, substance use, or co-occurring disorders in need of immediate placement in a safe environment with on-site supportive services. The goal of the HCHV Contract Residential Care Program is to rapidly stabilize Veterans medical, mental health, substance abuse and other psychosocial problems in order to place Veterans in other appropriate transitional or permanent housing 180 days. VA intends to engage a Contractor to provide rapid placement of Veterans in a safe, supportive, residential setting while addressing Veterans complex needs through on-site supportive case management services using a Safe Haven model. This housing must meet the criteria established both by HCHV clinical staff and VA fire and safety officials. The contractor shall provide the following services: Emergency Housing Facilities and Management: Contract must have site control of the housing facility through owner ship or valid lease. The facility must meet fire and safety code imposed by the state Law and the Life Safety Code of the National Fire and Protection Association. The facility must meet Americans with Disabilities Act (ADA) guidelines for accessible accommodations for Veterans with physical limitations or impairments. Veterans must not be segregated from the rest of the facility due to physical disability; they must have full access to the physical plant as well as the services and supports at the facility. The contractor providing LDSH housing and supportive services to female Veterans under this contract is required to ensure the safety and privacy of these Veterans. Men and women must have separate bathroom facilities. The facility must have female residential rooms or sections that are securely segregated or restricted from men to ensure safety and privacy. Contractor must provide a safe and sober environment for all residents that is consistent with the Safe Haven model of housing and services. Staffing and Supportive Services: The contractor shall employ sufficient professional health care personnel to carry out the policies, responsibilities, and programs of the facility 24 hours per day, seven days per week, 365 days per year including weekends and all state and federal holidays. Contractor staff must maintain professional boundaries with the Veteran at all times while conveying and attitude of genuine concern and caring. The contractor shall comply with the VA patient s Bill of Rights as set forth 38 CRF17.33 Patient s Rights and is responsible for maintaining Veterans privacy and confidentiality and must have systems in place that protect Veteran s personal identifying information and protected health information. Admissions, length of stay and discharges: the coal of the HCHV LDSH program is to rapidly stabilize Veterans medical, mental health, substance abuse and other psychosocial problems by expediting placement of these Veterans into safe, supportive emergency housing. And place Veterans in other appropriate transitional or permanent housing no more than 180 days from the date of admission. Special Qualifications: The contractor shall maintain a qualified work force to complete the work. 6. Please note that if VA s pending market research establishes that two or more VOSB or SDVOSB, who are certified in VA s Vendor Information Pages (VIP) site https://www.vip.vetbiz.gov/, can perform the services, VA will set aside the underlying solicitation per 38 U.S.C. §8127.   7. All Offerors who provide goods or services to the United States Federal Government must be registered in the System for Award Management (SAM) database found at https://www.sam.gov. Registration must include Representations and Certifications. 8.
