Description
COMBINED SYNOPSIS/SOLICITATION The VA Pittsburgh Healthcare System, has a requirement for MICROSCOPE SLIDES, SUPERFROST PLUS. The solicitation number is 36C24419Q0197 and it is issued as a Combined Synopsis, Request for Quotation (RFQ). The associated North American Industrial Classification System (NAICS) code for this procurement is 339112 with a small business size standard of 1000 employees. All items will be delivered to VA Pittsburgh Healthcare System, University Drive C, Shipping Dock, Pittsburgh, PA 15240. Successful offer will be the one that provides the Lowest Price Technically Accepted (LPTA). Please read the entire solicitation. Offerors are responsible for all terms, clauses, and conditions contained therein. This solicitation is set aside for Service Disabled Veteran Owned Small Businesses (SDVOSB) and requires registration with the System for Award Management (SAM) and Vendor Information Pages (VETBIZ) prior to award, pursuant to applicable regulations and guidelines. Registration information can be found at www.sam.gov and www.vip.vetbiz.gov. New equipment ONLY, NO remanufactured or used products. No "GREY" market items. Offer MUST be good for 30 calendar days after submission Quotes shall be submitted by email to Justin Erdley (Justin.Erdley@va.gov) Quotes are due by 12:00 PM, Eastern Time, on Tuesday 01-29-2019. FOB Destination, CONUS (Continental U.S.) Shipping charges shall be included in the purchase cost of the products. Sellers shall deliver the products on the own conveyance to the location listed on the purchase order. No partial shipments unless otherwise specified at time of order. Please see below and attached RFQ and Limited Sources Justification for additional information: (i) This is a combined synopsis/solicitation for brand name FisherBrand, Superfrost Plus Glass Adhesion Microscope Slides for the Pittsburgh Pathology and Laboratory, Medicine Service, Department of Veterans Affairs, as prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. A firm-fixed price purchase order is anticipated. (ii) The solicitation number is 36C24419Q0197 and is issued as a request for quotation (RFQ). (iii) The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-95. (iv) This solicitation is set aside 100% for Service-Disabled Veteran-Owned Small Businesses and the associated NAICS 339112 code has a small business size standard of 1000. The contractor shall provide glass slides that are specially treated to electrostatically adheres tissue sections and cells to the glass without the need for adhesives or protein coatings. The VA Pittsburgh Pathology and Laboratory Medicine Service is seeking the following reagents: The slides must electrostatically adheres tissue sections and cells to the glass without the need for adhesives or protein coatings. 50 boxes of 144 slides or 5 cases of 10 boxes will be delivered by the second Thursday of each month. Slides must have chemical-resistant white marking surface. Slides must be size: 25 x 75mm. Slides must be FisherBrand, Superfrost Plus. Freight charges are included in the price of the reagents. B. CONTRACTOR INFORMATION REQUIREMENTS: Contractor agrees to notify the Contracting Officer Representative (COR) of any changes to reagent composition, shipping delays or product recalls. Contractor will work with COR to schedule any required training or education. The Contractor will provide any updates to material data safety sheets (MSDS). C. GOVERNMENT FURNISHED PROPERTY: The government will furnish no property to the vendor. D. TERM OF CONTRACT: This will be a base year contract with 4 option years. No materials shall be sent by the contractor after the end date until the Contracting Officer authorizes such services in writing. E. LEGAL REQUIREMENTS RELATED TO MEDICAL RECORDS/PRIVACY AND CONFIDENTIALITY: Contractor must adhere to the provisions of Public Law 104-191, Health Insurance Portability and Accountability Act (HIPAA) of 1996 and the National Standards to Protect the Privacy and Security of Protected Health Information (PHI).   As required by HIPAA, the Department of Health and Human Services (HHS) has promulgated rules governing the security, use, and disclosure of protected health information by covered entities, including the Department of Veterans Affairs (VA).   Records created by the Contractor in the course of treating patients under this contract are the property of VA and shall not be accessed, released, transferred or destroyed except in accordance with applicable federal law and regulations. The treatment and administrative patient records created by, or provided to, the Contractor under this contract are covered by the VA system of records entitled Patient Medical Records VA (24VA19), which is available at http://www.rms.oit.va.gov/SOR_Records/24VA19.asp The Contractor is required to comply with Title 48 of the Code of Federal Regulations, Part 24.101-24.104 Protection of Privacy and Freedom of Information. When responding to a veteran s request that copies of records is released to a third party, the Contractor shall refer the veteran to VA for release of records. The Contractor is responsible for complying with VHA, Joint Commission, HIPAA, and other applicable external regulatory agency regulations. Neither the VA nor the Contractor has the legal authority to require that a patient provide his/her Social Security Number to the VA as a condition for receiving medical care under Title 38, United States Code. If the patient does not provide a Social Security Number, the VA will assign a unique identification number to the patient. F.
