Description
THIS IS NOT A SOLICITATION. This is a Request for Information (RFI)/ Sources Sought Notice only and is issued in accordance with Federal Acquisition Regulation (FAR) 10 Market Research, in order to conduct market research. Responses to this notice are not offers and cannot be accepted by the Government to form a binding contract. This RFI is issued solely for information and planning purposes and does not constitute a solicitation or a guarantee to issue a solicitation in the future. This RFI does not commit the Government to contract for any supply or service. All information submitted in response to this announcement is voluntary; the Government will not pay for information requested nor will it compensate any respondent for any cost incurred in developing information provided to the Government. Not responding to this RFI does not preclude participation in any future solicitation, if issued. This announcement is based on the best information available at this time and is subject to future modification. The North American Industry Classification System (NAICS) Code is 561320 Temporary Staffing, Small Business Size Standard $27.5 Million. Purpose and Objectives: The Department of Veterans Affairs (VA), Network Contracting Office (NCO) 2, and the Buffalo/Batavia VA Medical Center anticipates a Firm Fixed Price (FFP) procurement for a six (6) month base period with one (1) six (6) month option to extend services for Radiation Safety Officer/Laser Safety Officer Services. The Contractor shall ensure that the Radiation Safety Officer (RSO)/Laser Safety Officer (LSO) holds the required qualifications to perform on a Master Materials License (MML) and a Type A medical use broad scope radioactive materials license with research. Duties and Response times include, but are not limited to: Serves as the institution s Radiation Safety Officer and Laser Safety Officer Serves as a member of the institution s Radiation Safety Committee and prepares content for presentation at meetings and produces meeting minutes as required by VA regulation. Perform comprehensive quarterly audits of the radioactive material use areas provide written reports to the Radiation Safety Committee (RSC). Perform an annual review of the radiation safety program consistent with NRC requirements and provide a written report to the RSC and brief executive management of the results. Provide consultation and instruction regarding radiation safety to patients, staff, and for general public radiation safety. Review Qualified Medical Physicist s calculated and specified thickness, material, and placement of shielding needed to protect patients, workers, the general public and the environment from radiation produced incident to diagnosis or treatment of humans. Assess and evaluate installed shielding designed to protect patients, workers, and the general public from radiation produced incident to diagnosis or treatment of humans. Develop and monitor radiation protection equipment specifications. Develop and monitor procedures for the initial and continuing evaluation of radiation protection equipment and procedures. Monitor compliance with radiation protection, policies and procedures, regulations, accreditation organization(s) standards, and national recommendations. Evaluate radiation safety procedures prior to use. Develop, manage and evaluate the radiation safety program. Provide radiation-protection training for medical practitioners, and other health-care providers. Determine presence, evaluate, and assess any radiological hazard resulting from the use of ionizing radiation or radioactivity for compliance with appropriate regulatory and accreditation agencies. Shall remain up to date, at all times, of any change to requirements necessary as mandated by all regulatory agencies. Shall return all urgent phone calls within two (2) hours and non-emergent phone calls within twenty-four (24) hours. Also, be able to report, on-site, to respond to incidents/emergencies within eight (8) hours when the VAWNYHS deems the PSO s presence is essential. Participate in other Medical Center committee meetings besides the Radiation/Laser Safety Committee, when requested, when necessary regarding radiation safety issues. Participate in regulatory inspections (e.g. OSHA/JC, US NRC, VA NHPP) of the Medical Center. Document radiation safety training of the Assistant Health Physicist by submitting an updated US NRC Form 313A (See the above identified US NRC link) to the Medical Center Chief of Staff on a quarterly basis. The Laser Safety Officer is responsible for administering a Laser Hazard Control Program. Recommend to the Medical Center Director approval or disapproval of all laser purchases, laser use and storage locations, standard operating procedures, credential criteria and training programs for laser personnel. Hold all training LSO qualifications current, and provide documentation upon quote and then as requested. Maintain a roster of all personnel at risk from exposure to laser radiation. Maintain a current inventory of all Class 3b and 4 lasers and their locations. Ensure laser equipment and installations are properly classified and labeled. Investigate and immediately submit a notification message to the Medical Center Safety Manager of any actual or suspected laser exposure incident.
