- VETERANS AFFAIRS MEDICAL CENTER, PHILADELPHIA, PA REQUEST FOR QUOTE FOLLOWED BY TENTATIVE STATEMENT OF WORK Veterans Health Affairs, Network Contracting Office 4, is issuing this Request for Quote (RFQ) in order to identify capable firms to provide Laerdal NURSING ANNE SIMULATOR EQUIPMENT for the Philadelphia, PA VA Medical Center. THIS RFQ is BRAND NAME OR EQUAL TO. Responses must be submitted by February 1, 2019 by 9:00AM Eastern Standard Time (EST). Submit responses to the information requested above via email to rashaun.thomas@va.gov. All SDVOSB and VOSB firms that respond shall include proof of CVE certification via www.vip.vetbiz.gov. All small business firms that respond shall include proof of small business status via their Representations and Certifications in accordance with (FAR 4.1102 Policy). All firms that respond must be registered in the System for Award Management via https://sam.gov/SAM/. All firms who respond must submit their Duns & Bradstreet number via https://www.dnb.com. Any firms holding an GSA schedule contract from https://www.ebuy.gsa.gov/advantage/ebuy/start_page.do must submit the GSA contract number. All firms who respond must submit their tax ID number. While SDVOSB/VOSB contractors are preferred, all capable contractors are welcome to respond to this source sought notice for market research purposes, and shall include as part of their response a brief capability statement that covers the information in the following tentative statement of work. The results of this market research will assist in the development of (1) the requirement, and (2) the acquisition strategy (e.g., socioeconomic set-aside, full and open competition, etc.). VA assumes no responsibility for any costs incurred associated with the preparation of responses. This RFQ will accept any recommendations for Product Service Code or NACIS codes. All vendors shall use the price schedule located in section B to quote the price for each line item. Attachments are acceptable, each vendor MUST fill out the information requested in pricing schedule located in section B for each individual line item. TENTATIVE STATEMENT OF WORK Objectives: The overall goal of simulation is to provide education and deliberate practice opportunities to the learner in a safe, non-judgmental learning environment. The Nursing Anne simulator supports nursing education and training in the following areas; orientation, health assessment, medical surgical skills and team training, annual competency assessment, and nutrition. The Nursing Anne Simulator specifications include and shall adhere to the following requirements: Full Body Manikin with articulating arms and legs. Includes Brunette Wig, Brown Pupil Set, Ostomy Set, Blood Pressure Cuff, Manikin Lubricant Spray, Simulated Blood, Adult Gown and Quick set up guide. Medication: Administration of eardrops Oral via NG tube Bilateral pre-ported IV arms with capability for intravenous bolus or push through intravenous infusion when attached to a fluid bag Optional female multi-venous IV training arm with capability of IV cannulation placement Bilateral deltoid, ventral gluteal, dorsal gluteal and thigh injection sites Palpable anatomy to aid in site selection includes anterior superior iliac crest, pubic symphysis and greater trochanter Vaginal canal allows for insertion of vaginal suppositories or medications Anal opening will accept real and simulated rectal suppositories Hygiene: Wig for hair care procedures Ear canal for practice of irrigation and cleaning Oral care and hygiene Removable upper denture for oral and denture care Bed baths and skin care Patient simulator shall allow manipulation into dorsal recumbent position for perineal care Perineal care, including separation of the labia for cleansing Skin Integrity and Wound Care: Patient positioning for prevention of pressure ulcers Bandage and binder application Nasal packing Dressing and dressing changes Toes spread for bandaging Patient Handling: Full articulation for realistic patient handling procedures Head can be flexed into chin to chest position and remain flexed until repositioned Sits unassisted and bends at waist to tripod position Protective positioning Patient transfer techniques Range of motion exercises Nutrition: Accurate anatomical landmarks for insertion of NG Tube to correct measurement (nose to earlobe to xiphoid process) Lavage and gavage Invisible port in upper left abdomen for pre-placement of a PEG or J tube for feeding Internal reservoir with 500mL fluid capacity, external quick fill port, and optional external bag for large volume feeding.
