- Sources Sought Notice This Sources Sought/Market Survey Notice is being conducted by the Network Contracting Office (NCO) 16 at 2002 Holcombe Blvd. Bldg. 100, Houston, Texas 77030 to identify other sources that are capable of providing air quality monitoring and analysis for asbestos airborne concentrations during asbestos removal project located at the Central Arkansas Veterans Healthcare System (CAVHS) at the John L. McClellan Memorial Veterans Hospital located at 4300 West 7th Street, Little Rock, Arkansas 72205. Contractor shall be able to perform services at hundred percent. Contractor shall furnish all labor, material, tools, equipment, and supervision necessary to perform the air quality monitoring and analysis for asbestos airborne concentrations during asbestos removal project located at the Central Arkansas Veterans Healthcare System (CAVHS) at the John L. McClellan Memorial Veterans Hospital as specified in the contract. Draft Statement of Work (a) GENERAL GUIDANCE: (1) Title of Project: Air Quality Monitoring during Asbestos Removal. (2) Scope of Work: The purpose of this contract is to provide air quality monitoring and analysis for asbestos airborne concentrations during asbestos removal project located at the Central Arkansas Veterans Healthcare System (CAVHS) at the John L. McClellan Memorial Veterans Hospital, Little Rock, Arkansas. (3) Background: The present flooring substrate is buckling and presents a tripping hazard to employees. The mastic underneath the floor tile contains asbestos. Tiles, mastic and affected flooring substrate shall be removed as Asbestos Containing Materials (ACM) before any future renovation can take place. VHA Directive 7714 prohibits contractors performing abatement to perform, or sub-contract the performance of final clearance air monitoring and visual inspection. (b) DEFINITION/ACRONYMS: (i) ASBH Arkansas State Board of Health (ii) CAVHS Central Arkansas Veterans Healthcare System (iii) COR Contracting Officer s Representative (iv) CO Contracting Officer (v) FAR Federal Acquisition Regulation (vi) OSHA Occupational Safety and Health Administration (vii) SOW Statement of Work (viii) ADEQ Arkansas Department of Environmental Quality (ix) EPA Environmental Protection Agency (x) ACM Asbestos Containing Materials (xi) NESHAP National Emission Standards for Hazardous Air Pollutants (xii) APC&EC Arkansas Pollution Control and Ecology Commission (xiii) CIH Certified Industrial Hygienist (c) GENERAL REQUIREMENTS: (1) Furnish all labor, material, tools, equipment, and supervision necessary to perform pre-abatement air monitoring and analysis for background asbestos airborne concentrations. (2) Furnish all labor, material, tools, equipment, and supervision necessary to perform daily air monitoring and analysis (up to 5 days) outside the abatement enclosure for asbestos airborne concentrations. (3) Furnish all labor, material, tools, equipment, and supervision necessary to perform final clearance air monitoring for asbestos airborne concentrations, along with final clearance visual inspection of the abatement area. (ii) Work site: Department of Veterans Affairs Medical Center, Building 41, North Little Rock, AR. (4) The contractor shall keep at the site: (i) A copy of the inspection report, including results of any bulk sample analysis, and any air monitoring data, (ii) A copy of certifications and licenses of personnel participating in air monitoring actions. (5) The ACM removal shall be completed in accordance with protocols mandated for such activities by EPA, NESHAP, and ADEQ regulations for asbestos management in hospitals and OSHA regulations. The removal and monitoring shall be conducted by person(s) certified by the ADEQ to conduct such activities for a school entity. (6) Contractor shall provide two (2) copies of a written report that details result of air monitoring and inspection that meets EPA and ADEQ requirements for such a report for a school entity. The report copies shall be provided to the abatement contractor and Engineering Service at the following address: Central Arkansas VA Healthcare System Engineering Service 138 Attn: Safety Office 2200 Fort Roots Drive, Bldg. 1 North Little Rock, AR 72114 (7) All dimensions listed above are approximate and determination of exact quantities and dimensions, as needed, remains the responsibility of the Contractor. (8) All activities shall be conducted in an appropriate and safe manner that protects VA employees, patients, and visitors from any potential exposure to airborne asbestos. Contractor firm is responsible for the health and safety of their own employees and shall take all precautions deemed necessary. The Contractor shall take precautions deemed appropriate and provide any necessary equipment to their employees in order to take these precautions. (9) Work shall be performed between 7:00 a.m. and 5:00 p.m. Monday-Friday including weekends (if necessary). Changes to work hours shall be approved by the project COR. (10) All work is to be accomplished and reports submitted to the VA Representative within thirty (30) calendar days after notice to proceed. The work schedule shall be mutually agreed upon with the VA Representative and the Contractor. A schedule of work shall be provided by the Contractor to the VA Representative prior to start of work. (11) Parking shall be limited to established parking lots. Contractor equipment, vehicles, and personnel are not allowed on any grass or non-paved areas unless specifically authorized by the VA Representative. U.S. District Court Violation Notices shall be issued for parking violations. The contractor and all employees shall comply with all applicable VA Medical Center fire, safety, and traffic regulations. (12) The Contractor shall be responsible for the replacement and/or repair of any damage incurred by the Contractor s employees during the project. (13) Contractor shall keep work area clean. No tools, materials or debris shall be left unattended. Any debris shall be immediately removed and may not be left in trash cans within the medical center. (14) All Contractor employees shall wear the appropriate Medical Center identification badge issued to them at all times in the manner required. (15) Work on this project shall not begin until authorized by the Contracting Officer. (16) The title VA Representative shall refer to the Contracting Officer s Representative (COR) located at the CAVHS Engineering Office. (17) All personnel air monitoring results shall be provided in a timely manner to prevent any Project delays. (d) CONTRACTOR QUALIFICATIONS: (1) Firm shall demonstrate proficiency in performing asbestos air monitoring by: (i) Minimum of five years in asbestos air monitoring, and shall meet Arkansas licensures and certifications for air monitors. APC&EC Regulation 21 requires that individuals who work in asbestos abatement be trained and certified. Air monitors, contractor supervisors, inspectors, management planners, project designers, and workers certified for asbestos work in Arkansas. (ii) Provide references of previous ACM air monitoring. (iii)Provide listing of team members that shall be assigned to this project with individual qualifications and responsibilities of team members. (2) This project shall be under the direct supervision of a CIH who shall be employed by the firm. The CIH shall have active involvement and shall visit the site at least weekly to discuss the project status with the designated VA representative COR. (e) GOVERNMENT PERSONNEL OBSERVED HOLIDAYS: (1) The Government hereby provides notice and Contractor hereby acknowledges receipt that Government personnel observe the listed days as holidays: (i) New Year's Day January 1 (ii) Martin Luther King's Birthday Third Monday in January (iii) President's Birthday Third Monday in February (iv) Memorial Day Last Monday in May (v) Independence Day July 4 (vi) Labor Day First Monday in September (vii) Columbus Day Second Monday in October (viii) Veterans Day November 11 (ix) Thanksgiving Day Fourth Thursday in November (x) Christmas December 25 (2) In addition to the days designated as holidays, the Government observes the following days: (i) Any other day designated by Federal Statute (ii) Any other day designated by Executive Order (iii) Any other day designated by the President s Proclamation (3) It is understood and agreed between the Government and the Contractor that observance of such days by Government personnel shall not otherwise be a reason for an additional period of performance, or entitlement of compensation. In the event the Contractor's personnel work during the holiday, they may be reimbursed by the Contractor, however, no form of holiday or other premium compensation shall be reimbursed either as a direct or indirect cost, other than their normal compensation for the time worked. End of Draft Statement of Work The expected period of performance is for a one-time service for when the asbestos removal services are schedule; possible scheduling from April 15, 2019 thru June 30, 2019.
