This announcement constitutes the only solicitation; quotes are being requested, and a written solicitation document will not be issued. The Government reserves the right to make no award from this solicitation. This solicitation is a Request for Quote (RFQ). The government anticipates awarding a fixed price indefinite delivery indefinite quantity contract resulting from this solicitation. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2019-01 Effective 20 Dec. 2018. This solicitation is set aside 100% for Veteran Owned Small Businesses. The associated North American Industrial Classification System (NAICS) code for this procurement is 541380 with a small business size standard of $15,000,000.00. The Department of Veterans Affairs, Network Contracting Office 4, 1010 Delafield Road, Pittsburgh, PA 15215 is soliciting offers from Small Veteran Owned Businesses to provide RADIATION PHYSICIST SERVICES for the Wilkes Barre Veteran Affairs Medical Center, Allentown Outpatient Clinic, and Sayre Outpatient Clinic. Respondents to this solicitation must fully demonstrate their capability by supplying detailed quote information along with any other documents necessary to support the requirements below. All interested companies shall provide quotations for the following Schedule of Services: Schedule is attached as a separate document. (A1) Services: Performance Work Statement is attached as a separate document. (A2) The period of performance is: five years as follows: Year 1: April 01, 2019 through March 31, 2020 Year 2: April 01, 2020 through March 31, 2021 Year 3: April 01, 2021 through March 31, 2022 Year 4: April 01, 2022 through March 31, 2023 Year 5: April 01, 2023 through March 31, 2024 Place of Performance: Address: Wilkes Barre Veterans Affairs Medical Center (WBVAMC) located at 1111 East End Boulevard, Wilkes Barre, PA Allentown Outpatient Clinic (OPC) located at 3110 Hamilton Boulevard, Allentown, PA Sayre Outpatient Clinic (OPC) located at 1537 Elmira Street, Sayre, PA. The Government will select the quote that represents the best benefit to the Government at a price that can be determined reasonable. The Government will evaluate price, technical capabilities, and past performance. Specific quote requirements are detailed below in ADDENDUM TO FAR 52.212-1 Instructions to Offerors Commercial Items. Following receipt of quotes, the Government will perform an evaluation using a comparative evaluation of the services quoted in accordance with FAR 13.106-2(b)(3). The Government will compare quotes to one another to select the quote that best benefits the Government by fulfilling the requirement. Contractors may submit more than one quote; however, contractors are strongly encouraged to submit their best technical solutions and prices in response to this request for quote. Once the Government determines there is/are a contractor(s) that can provide a service that meet(s) the requirements of this request for quote, the Government reserves the right to communicate with only those contractors quoting the best-suited service to address any remaining issues. The Offeror shall submit a capability statement demonstrating their technical capability, experience, and ability to meet the full scope of the performance work statement. The capability shall include information as to the key personnel and their qualifications to include proof that the Radiation Physicist: shall meet the following requirements: Possess at least a Bachelor s Degree in physics or a related field Have at least five (5) years experience performing radiation physicist duties in a health care facility Board certification in diagnostic radiologic physics or radiologic physics by the American Board of Radiology, or in Diagnostic Imaging Physics by the American Board of Medical Physics, or in Diagnostic Radiological Physics by the Canadian College of Physicists in medicine, or meet all of the following requirements: A graduate degree in physics, medical physics, biophysics, radiologic physics, medical health physics, or a closely related science or engineering discipline from an accredited college or university College coursework in the biological sciences with at least one course in biology or radiation biology and one course in anatomy, physiology, or a similar topic related to the practice of medical physics. Documented experience in a clinical CT environment conducting at least 10 CT performance evaluations under the direct supervision of a board certified medical physicist. iv. HHS/OIG: To ensure that the individuals providing services under the contract have not engaged in fraud or abuse regarding Sections 1128 and 1128A of the Social Security Act regarding federal health care programs, the contractor shall check the Health and Human Service Office of Inspector General, List of excluded individuals/entities on the OIG website (www.hhs.gov/oig) for each person providing services under the contract. Further the contractor shall be required to certify in its proposal that all persons listed in the contractor s proposal have been compared against the OIG list and are not listed. All contract employees must be United States citizens or permanent residents. Contractor personnel shall read, understand, speak, and write English well enough to effectively communicate with patients. Capability statements that are merely an overview of what the contractor offers will be determined incomplete. Completed Price Schedule (attached A2) to include Unit Price, Total Price, and Grand Total. Failure to provide complete pricing, including pricing for each CLIN, will result in the contractor s package being determined to be incomplete. The Offeror may submit up to three (3) past performance references, including both reference information and details of the work performance. The Contracting Officer may utilize all available records (e.g. PPIPRS and FAPIIS) as well to evaluate a contractor s past performance. Acknowledgement of any solicitation amendments A statement specifying the extent of agreement with all terms, conditions, and provisions included in the solicitation. Offers that fail to furnish required representation or information, or reject the terms and conditions of the solicitation, may be excluded from consideration. Such orders may be issued from April 1, 2019 through March 31, 2024. (b) All delivery orders or task orders are subject to the terms and conditions of this contract. In the event of conflict between a delivery order or task order and this contract, the contract shall control. (c) If mailed, a delivery order or task order is considered "issued" when the Government deposits the order in the mail. Orders may be issued orally, by facsimile, or by electronic commerce methods only if authorized in the Schedule. (End of Clause) 52.216-19 ORDER LIMITATIONS (OCT 1995) (a) Minimum order. When the Government requires supplies or services covered by this contract in an amount of less than 15,000.00, the Government is not obligated to purchase, nor is the Contractor obligated to furnish, those supplies or services under the contract. (b) Maximum order. The Contractor is not obligated to honor (1) Any order for a single item in excess of $50,000; (2) Any order for a combination of items in excess of $50,000; or (3) A series of orders from the same ordering office within 365 days that together call for quantities exceeding the limitation in paragraph (b)(1) or (2) of this section. (c) If this is a requirements contract (i.e., includes the Requirements clause at subsection 52.216-21 of the Federal Acquisition Regulation (FAR)), the Government is not required to order a part of any one requirement from the Contractor if that requirement exceeds the maximum-order limitations in paragraph (b) of this section. (d) Notwithstanding paragraphs (b) and (c) of this section, the Contractor shall honor any order exceeding the maximum order limitations in paragraph (b), unless that order (or orders) is returned to the ordering office within 60 days after issuance, with written notice stating the Contractor's intent not to ship the item (or items) called for and the reasons. Upon receiving this notice, the Government may acquire the supplies or services from another source. (End of Clause) 52.216-22 INDEFINITE QUANTITY (OCT 1995) (a) This is an indefinite-quantity contract for the supplies or services specified, and effective for the period stated, in the Schedule. The quantities of supplies and services specified in the Schedule are estimates only and are not purchased by this contract. (b) Delivery or performance shall be made only as authorized by orders issued in accordance with the Ordering clause. The Contractor shall furnish to the Government, when and if ordered, the supplies or services specified in the Schedule up to and including the quantity designated in the Schedule as the "maximum." The Government shall order at least the quantity of supplies or services designated in the Schedule as the "minimum." (c) Except for any limitations on quantities in the Order Limitations clause or in the Schedule, there is no limit on the number of orders that may be issued. The Government may issue orders requiring delivery to multiple destinations or performance at multiple locations. (d) Any order issued during the effective period of this contract and not completed within that period shall be completed by the Contractor within the time specified in the order. The contract shall govern the Contractor's and Government's rights and obligations with respect to that order to the same extent as if the order were completed during the contract's effective period; provided, that the Contractor shall not be required to make any deliveries under this contract after contract expiration. (End of Clause) 52.217-8 OPTION TO EXTEND SERVICES (NOV 1999) The Government may require continued performance of any services within the limits and at the rates specified in the contract. These rates may be adjusted only as a result of revisions to prevailing labor rates provided by the Secretary of Labor. The option provision may be exercised more than once, but the total extension of performance hereunder shall not exceed 6 months. The Contracting Officer may exercise the option by written notice to the Contractor within 15 days. *The specified rates under this clause will be those rates in effect under the contract each time an option is exercised under this clause.
