Page 3 of 3 THIS IS A SOURCES SOUGHT NOTICE REPRESENTING A MARKET SURVEY AND IS NOT A REQUEST FOR PROPOSALS, PROPOSAL ABSTRACTS, QUOTATIONS OR INVITATION FOR BIDS The Department of Veterans Affairs, Augusta GA, is conducting a MARKET SURVEY to obtain information regarding: the availability and capability of qualified Service Disabled Veteran Owned Small Businesses (SDVOSB). Your responses to the information requested will assist the Government in determining the appropriate acquisition method, including whether a set-aside is possible. An organization that is not considered an SDVOSB under the applicable NAICS code should not submit a response to this notice. Eligible concerns should be capable of furnishing all labor, supervision, and parts, to include all equipment necessary to perform all work required under the proposed contract. Industry responses are being solicited as part of the market research process. The Department of Veteran Affairs, Network Contracting Office (NCO 7), intends to issue a solicitation, for Home Oxygen Services, for multiple VA locations within the VISN 7 geographic area. The following VA VISN 7 locations, their associated outpatient clinics, and their entire patient care catchment areas, will be included in the resultant contract: The Department of Veteran Affairs (VA) Medical Center in Atlanta, GA currently serves an estimated 1250 oxygen-using patients. The Department of Veteran Affairs (VA) Medical Center in Augusta, GA currently serves an estimated 615 oxygen-using patients. The Department of Veteran Affairs (VA) Medical Center in Birmingham, AL currently serves an estimated 800 oxygen-using patients. The Department of Veteran Affairs (VA) Medical Center in Montgomery, AL currently serves an estimated 500 oxygen-using patients. The Department of Veteran Affairs (VA) Medical Center in Charleston, SC currently serves an estimated 689 oxygen-using patients. The Department of Veteran Affairs (VA) Medical Center in Columbia, SC currently serves an estimated 1250 oxygen-using patients. The Department of Veteran Affairs (VA) Medical Center in Dublin, GA currently serves an estimated 380 oxygen-using patients. The Department of Veteran Affairs (VA) Medical Center in Tuscaloosa, AL currently serves an estimated 154 oxygen-using patients. In accordance with the Performance Work Statement, the Contractor shall furnish all supplies, materials, equipment, and personnel necessary for patients requiring home oxygen services. Contractor shall provide Certified or Registered Respiratory Therapists to train, educate, and monitor patients, as well as complete assessments of the beneficiary s home and environment. Respiratory Therapists shall be licensed in accordance with the state in which they provide service. VISN 7 has beneficiaries that reside primarily Alabama, Georgia, and South Carolina. Contractor must be accredited by Joint Commission (JC) or another national accreditation body that meets or exceeds Joint Commission standards, and must maintain accreditation throughout the duration of the contract. There are approximately 5,638 active patients at this time. The services associated with this contract include: All logistical/administrative functions involved with the day to day operation of providing the service to the Veteran, including in home respiratory therapy visits. This includes coordinating and managing patient care, deliveries and set-ups, briefing patient on proper use of equipment, documenting use of supplies and inspection of home in accordance with JC standards concerning patient use/safety of home oxygen. The solicitation will be issued on the Federal Business Opportunities (FBO) website (https://www.fbo.gov) on/about March 1, 2019. The North American Industry Classification System (NAICS) Code is 532283-Home Health Equipment Rental with a size standard of $32.5 Million. In order to be considered for award, prospective Offerors are reminded that in accordance with FAR 4.11 they shall be registered in the System for Award Management (SAM) database and shall complete electronic annual representations and certifications in SAM prior to award of a contract. SAM is located at http://www.sam.gov and consolidated the capabilities of CCR/FedReg, ORCA, and EPLS. There is NO cost to use SAM. This notice is for informational purposes only and is not a request for submission of proposals. Capability statements, locations of interest, and business size information is requested. Submit the following information not later than February 13, 2019 at 4:30 EST. POTENTIAL SOURCES SHALL PROVIDE THE FOLLOWING INFORMATION IN THEIR RESPONSE: 1) Company name, address, phone number, primary contact(s), e-mail address, NAICS code(s), business size (i.e., small/large) and DUNS Number. 2) Provide a Statement of Capability that demonstrates the offeror's past performance in providing this type of service. Please include the following: (a) staff expertise including their availability, experience, and formal and other training; (b) current in-house capability and capacity to perform the work; (c) prior completed projects of similar nature and scope; (d) corporate experience and management capability; and (e) examples of prior completed Government contracts, references, and other related information. (f) provide any certifications obtained for accreditation of facilities and processes. 3) Identify the area, or areas of interest and describe the capability to serve the catchment area. NOTICE: OFFERORS MUST HAVE A BUSINESS LOCATION PRIMARILY ENGAGED IN PERFORMANCE OF THE SCOPE OF WORK FOR THE CATCHMENT AREA WITHIN A 50 MILE RADIUS FROM THE SUPPORTED MEDICAL CENTER.
