Page 1 of 1 SPECIAL NOTICE INTENT TO SOLE SOURCE Department of Veterans Affairs, Sierra Pacific Network Contracting Office (NCO 21), intends to negotiate a sole source contract with Mills-Peninsula Senior Focus Incorporated. This procurement is being conducted in accordance with FAR 13.501 in accordance with 41 U. S.C. 1901 only one responsible source and no other supplies or services will satisfy the agency requirements. The contract will be firm-fixed-price, and the NAICS Code is 624120. In accordance with 358 U.S.C. 1720 the Department of Veterans Affairs requires the contractor to provide the San Francisco VA Health Care System (SFVAHCS) with Adult Day Health Care Services in accordance with the Performance Work Statement (PWS) in San Mateo, California. This service is under the authority of Title 38 United States Code (U.S.C.), Section 1720 (f) and Title 38 Code of Federal Regulations (CFR), Section 17.38(a)(1)(xi)(B). Through this contract, the resultant contractor would be able to provide adult day health care for an estimated 80 Veterans per week. Upon acceptance of a VA patient into the program, when requested by the Contracting Officer or authorized representative, on an as needed basis, and at the per diem and length of time agreed upon, the contractor shall provide services in accordance with the Performance Work Statement. Interested vendors must provide evidence of a successful facility inspection by the VA s multi-disciplinary team within the last 12 months. This Notice of Intent is not a request for competitive proposals. If a firm believes it can meet the requirements it must furnish information about its products and services, as well as references from other customers who are using these products and service, to the Contracting Officer no later than 11:00AM PST, February 23, 2018. Supporting evidence must be furnished in sufficient detail to demonstrate the ability to comply with the above requirements. Information must be sent to the Contracting Officer Brian Trahan at brian.trahan@va.gov. Verbal responses will not be considered. Responses received will be evaluated however; a determination by the Government not to compete the proposed procurement based upon responses to this notice is solely within the discretion of the Government. If no responses are received, the Contracting Officer will proceed with the sole source negotiation with Mills-Peninsula Senior Focus Incorporated.
