Description
The Department of Veterans Affairs, Network Contracting Office 9, intends to award a sole-source, award a contract to Terumo Cardiovascular Systems Corporation, 6200 Jackson Rd. Ann Arbor, MI 48103 for the Memphis VA Medical Center, Memphis TN. The contract is expected to be awarded under FAR 13.106-1 (b). Period of Performance: Base Year: May 1, 2019 to April 30, 2020 Option year one: May 1, 2020 to April 30, 2021 Option Year two: May 1, 2021 to April 30, 2022 Option Year three: May 1, 2022 to April 30, 2023 Option Year four: May 1, 2023 to April 30, 2024 NAICS code: 811219, SBA Size Standard $20.5 Million. PSC: Q502. Terumo Cardiovascular Systems Corporation, shall furnish all necessary travel, labor, materials, tools, documentation, and parts including expendable and control components (where applicable) required for the shared support of the systems listed in the Schedule of Equipment, located at the Veterans Affairs Medical Center, 1030 Jefferson Ave., Memphis, TN 38104. Support shall be provided via on-site visits. This contract shall be performed during the designated hours of coverage, in accordance with all terms, conditions, provisions, schedules, and specifications of this solicitation. Contractor must have proven knowledge of all of the database software and hardware components and must demonstrate, to the satisfaction of the VAMC, that he/she is able to meet the requirement as outlined below. Vendor will provide parts, labor, and travel charges for two Periodic Maintenance calls and all emergency calls per year for the listed equipment below. Contractor will provide service documentation within 24 hours after the services are completed. Manufacturer s specifications, Association for the Advancement of Medical Instrumentation (AAMI), Joint Commission for the Accreditation of Healthcare Organizations (JCAHO), HOURS OF COVERAGE Normal VAMC working hours are Monday through Friday, 8 a.m. to 5p.m., excluding federal holidays. Federal Holidays observed by the VAMC are: New Years' Day, Martin Luther King Day, Presidents' Day, Memorial Day, Independence Day, Labor Day, Columbus Day, Veterans' Day, Thanksgiving Day, Christmas Day All service/repairs shall be performed during normal hours of coverage, with approval of the COR. Work performed outside the normal hours of coverage at the request of FSE will be considered service during normal hours of coverage. Normal hours of coverage for this contract are: Monday through Friday from 8 am to 5 pm, excluding federal holidays. Contractor must provide the COR with the means to request and receive prompt emergency telephone support from an FSE, within 2 hours of the COR s request. This emergency telephone call back service shall be provided / twenty-four hours a day, seven days a week at no extra charge to the Government. National Fire Protection Agency 99 (NFPA-99), Center for Device and Radiological Health (CDRH), Original Equipment Manufacturer (OEM), American Hospital Association (AHA), Institute of Electrical And Electronic Engineers (IEEE), Occupational Safety and Health Administration (OSHA), Memphis VAMC standard operating procedures, and any other Federal, State, and Local regulations mandated. THIS NOTICE OF INTENT IS NOT A REQUEST FOR COMPETITIVE QUOTES. However, all responsible sources may submit a capability statement or quotation, which shall be considered by the agency on or before February 28, 2019, 12:00pm Central Standard Time to david.milner@va.gov. A determination not to compete this requirement, based upon responses to this notice, is solely within the discretion of the Government. Information received will normally be considered solely for the purpose of determining whether to conduct a competitive procurement. The Department of Veterans Affairs will NOT be responsible for any costs incurred by interested parties in responding to this notice of intent. Only written responses will be considered. All responses from responsible sources will be fully considered. As a result of analyzing responses to this notice of intent, the Government shall determine if a solicitation will be issued. Any prospective contractor must be registered in the System for Award Management (SAM) in order to be eligible for award. Interested parties are encouraged to furnish information by email only with RESPONSE TO INTENT TO SOLE SOURCE TERUMO CARDIOVASCULAR SYSTEMS CORPORATION in the subject line. The capability documentation should include general information and technical background describing in detail the prospective contractor's capabilities and experience that will demonstrate the ability to support these requirements, including proof of training and certification for performing maintenance and service on the listed equipment and systems.
