- THIS IS A SOURCES SOUGHT NOTICE ONLY. THIS IS NOT A REQUEST FOR PROPOSAL. The Muskogee VA Medical Center located in Muskogee, OK and the Network Contracting Office (NCO) 19 are performing market research to determine if there is sufficient number of qualified providers/firms for courier services. Socio-economic precedence are as follows: (1) Service Disabled Veteran Owned Small Business; (2) Veteran Owned Small Business; or (3) Small Business to set aside GENERAL REQUIREMENTS Firms responding shall specify they can meet the required specifications: Provide all equipment, parts, service, and maintenance necessary for reagent / instrument rental operation of the equipment for Immunohistochemical staining. Equipment Operational features: 1. Function as a stand-alone automated immunhistochemistry/in Situ Hybridization instrument for staining of anatomic pathology fixed tissue slides. 2. Must have a 30-slide capacity with capability of at least 35 reagent slots. 3. The instrument must be able to perform both immunochistochemical (IHC) and In-Situ hybridization (ISH) on the same platform. 4. Must be able to regulate the temperature of each individual slide during antigen retrieval and antibody testing protocols. 5. Must be able to perform antigen retrieval on the same platform as IHC testing. 6. Instrument must be able to process following specimens Specimen #specimens per year Lung 50 Lesion 150 Colon 20 Prostate 100 Thyroid 10 Bone marrow 20 Nodes 15 Esophagus 20 Testicle 5 Breast 5 All interested parties who can meet the requirements stated above should respond to this announcement via email to Shawna.baumert@va.gov no later than Thursday, February 21, 2019 at 2:30 PM (Mountain Standard Time). Interested parties should provide the following: Company Name, Company Address, Point of Contact, phone number and email address, DUNS number and capability statement for the services as stated above. Additionally include; two (2) customer references to which you have provided similar services, and your company business size indicate which small business category for which you qualify. THIS IS NOT A REQUEST FOR PROPOSAL. NAICS Code 334516 is applicable to this acquisition, and the business size standard for a contractor is 1,000 employees. Interested firms are reminded that in accordance with FAR 4.12, prospective contractors shall complete electronic annual representations and certifications in conjunction with FAR 4.11 required registration in the System Award Management System (SAM) database prior to award of a contract.
