Description
THIS IS A SOURCES SOUGHT NOTICE (a) The Government does not intend to award a contract on the basis of this sources sought notice or to otherwise pay for the information solicited. (b) Although proposal, offeror, contractor, and offeror may be used in this sources sought notice, any response will be treated as information only. It shall not be used as a proposal. (c) Any information received from a contractor in response to this sources sought notice may be used in creating a solicitation. Any information received which is marked with a statement, such as proprietary or confidential, intended to restrict distribution will not be distributed outside of the Government, except as required by law. (d) This sources sought notice is issued for purpose of collecting information about the availability of authorized distributors of Welch Allyn to provide requirement for Vital Signs Units and using a CPRS integration/interface. VASNHS has reached end of life (EOL) on the current Vital Signs Units. This is to replace those EOL units, provide an integration into CPRS with the new units, and provide clinical training. This is a sources sought notice only, no formal solicitation document exists at this time. Offerors response shall include the following information: - Reference Number 36C26119Q0297 - Company Name, point of contact, title, phone number and email address. - DUNS number - Type of business (Small or large) and socioeconomic status whether Service Disabled Veteran Owned, Veteran Owned, Hubzone, 8(a), Woman owned, Small Disadvantaged, etc. or VOSB company to provide supplies for blood culture analysis. - State if your company has any government contracts against which this solicitation could be ordered (e.g., GSA, SEWP, NAC, GSA BPA, etc.) or if your proposal is submitted as a open market quote. For any government contract, provide your contract number, terms and conditions; SIN the products fall under (if any), delivery schedule and contract expiration date. - Contractors shall identify the NAICS code for the product being offered as well as their size status. - Contractors shall include the terms and conditions of the warranty for all proposed equal products. (e) Contractors that feel they have an equal product are encouraged to email full information to carol.lam@va.gov. Please email all inquiries by February 25, 2019. Only emailed received directly from interested persons are acceptable. Receipt must be acknowledged on my desktop computer to be recognized as received. Alternate Item submission: Offerors submitting alternate items must provide technical information sufficient for a lay person to determine acceptability. Information will not be paid for by the government nor returned to the supplying party. The information will be used to determine product acceptability. Lack of sufficient information for our staff to determine product acceptability will result in a negative determination. Any quote will then be found non-responsiveness to the solicitation and the quote will not be considered. New Equipment ONLY; NO remanufactured, used/refurbished or "gray market" items. All items must be covered by the manufacturer's warranty. No product in development shall be considered. Authorized dealers: Experienced firms only who are authorized distributor will be considered in addition to the original equipment manufacturer. A letter from the manufacturer stating your company is an authorized dealer for the line items must be in possession by the due date and be available for submission, if requested. Offerors shall be registered in the System for Award Management (SAM) prior to award. Registration information is located at www.sam.gov. Item Description/Part Number: Specific mandatory deliverables, tasks, salient characteristics: 1. Welch Allyn Vital Signs: a. 250 Vital Signs Units with SpO2, Temperature, Non-Invasive Blood Pressure, Pulse Rate, Mean Arterial Pressure (MAP), Custom Scoring, Internal Wireless Radio 802.11 a/b/g, FIPS 140-2 certified; b. 250 Cable Management Mobile Stand and Storage Bin; c. 250 HS-1M 2D Barcode Scanner with Coiled USB; d. 250 Device Connectivity Licenses 2. Connex Server Application Software Server: a. Two (2) Copies of Server Gateway Software (Production and Test) b. One (1) SQL Enterprise 2012 License (Production) c. Installation and Configuration (project Management, FSE Support, Apps Eng. Support, Clinical Support and Training); 3. CPRS/VistA Capability: a. Vital Sign Readings must be able to be sent wirelessly or LAN Connection; b. Data from Vital Signs must interact with a Software Server that is approved for connection between Hospital ADT Server and CPRS/VistA Server; c. Transmission of data between ADT or EMR to the Vital Sign Software Server must utilize HL7 messages; d. Vital Sign Software Server must be capable of being virtualized; e. Vital Sign Data must seamlessly integrate within CPRS/VistA to fill outpatient health record Delivery to: VA So. Nevada Healthcare System VA Warehouse/Logistics 5406 E.
