- INTRODUCTION This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. The applicable NAICS code is 541990 and the Small Business Size Standard is $15,000,000. To be considered for an award of a federal contract offerors must be registered in System for Award Management (SAM) at http://www.sam.gov. All SAM records must be complete and not missing elements such as representations and certifications. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-99 06-15-2018 & 07/15/2018. DESCRIPTION The contractor shall provide all personnel, equipment, tools, materials, supervision, transportation, and other items and services necessary to perform Annual Medical Gas Testing and Certification service for the CNVAMC, Uptown and Downtown locations in accordance with the attached Statement of Work (SOW). Contractor shall certify all medical outlets, zone valves, and alarms. Per code requirements the medical gas system is to be tested and certified annually for the two-division facility. Vendor shall provide all equipment and material to test and certify all medical gas outlets, zone valves and alarms. Contractor shall provide a complete report after completion. Schedule. This is a request for a 12-month Base and four one-year Option Periods. Contract Line Item Numbers, quantities, unit of measure as follow: CLIN 0001. Medical Gas Testing and Certification Services for Charlie Norwood VAMC Uptown and Downtown Divisions, 1 JOB, Base Period, Period of Performance 6/1/2019 to 07/31/2020; CLIN 1001. Medical Gas Testing and Certification Services for the Charlie Norwood VAMC Uptown and Downtown Divisions, 1 JOB, Option Year 1, Period of Performance 6/1/2020 to 07/31/2021; CLIN 2001. Medical Gas Testing and Certification Services for the Charlie Norwood VAMC Uptown and Downtown Divisions, 1 JOB, Option Year 2, Period of Performance 6/1/2021 to 07/31/2022; CLIN 3001. Medical Gas Testing and Certification Services for the Charlie Norwood VAMC Uptown and Downtown Divisions, 1 JOB, Option Year 3, Period of Performance 6/1/2022to 07/31/2023; and CLIN 4001. Medical Gas Testing and Certification Services for the Charlie Norwood VAMC Uptown and Downtown Divisions, 1 JOB, Option Year 4, Period of Performance 6/1/2023 to 07/31/2024. Place of Performance. Charlie Norwood VA Medical Center Uptown Division at 1 Freedom Way, Augusta GA 30904 and Charlie Norwood VA Medical Center Downtown Division at 950 15th Street, Augusta GA 30901. Hours of Operation. Work shall be accomplished between 7:30a.m. 3:30p.m., Monday through Friday, excluding weekends and Federal Holidays unless otherwise indicated. See Attached Statement of Work (SOW) for additional information. BASIS OF AWARD The Government intends to award a firm-fixed price contract to the responsible Offeror whose proposal, conforming to the solicitation, is the lowest price and conforms to all technical and past performance standards established below. Technical. Offerors must provide: Documentation of business Medical Gas System Certification Business License A list of personnel employees employed performing this type of service including years of experience. Past Performance. Offerors shall submit at least three references and three contracts demonstrating successful performance in Medical Gas Systems and Alarms Inspection and Certifications. Offerors must send the Past Performance Questionnaire (PPQ) that is attached to this solicitation to its references, and ensure its references submit a completed PPQ to lasonja.harvey@va.gov by the solicitation deadline. Offerors are not to send the PPQs directly to the Contracting Office. The Government will evaluate quotes for award purposes by adding the total price for all options to the total price for the basic requirement. The Government may determine that a quote is unacceptable if the option prices are significantly unbalanced. PROVISIONS AND CLAUSES This solicitation incorporates provisions and clauses in effect through Federal Acquisition Circular 2005-101. Descriptions for the occupations for this requirement and determine the appropriate wage determinations are the responsibility of the contractor, and offeror are encouraged to coordinate with Department of Labor in order to determine the appropriate job classifications for this requirement. The Agency assumes no responsibility or liability for a contractor's determination of the appropriate classification. The first 3 completed PPQs received will be used for evaluation purposes and any additional PPQs received for the same Offeror will be discarded without consideration.
