Description
The New York Army National Guard requires medical surveillance exams/ occupational health services at 10 different work sites/locations as specifed in the attached performance work statement. Approximately 350+ soldiers need the above mentioned exams. Dates: 9 April 2019- 1 March 2019 (as specified in PWS). Performance Locations: Throughout New York State to include Jamestown, Buffalo, Rochester, Fort Drum, Syracuse, Binghamton, Latham, Camp Smith, Staten Island, and Farmingdale Exams required: Eyesight, hearing, physicals, lead testing. Additional details specified in PWS attached. Set-Aside: Total Small Business The applicable wage determination is attached. Contractor Manpower Reporting Applies to this requirement and awarded contractor is required to input total labor cost on http://www.ecmra.mil/ once annually.
