RFQ 36C25519Q0233 Page 1 of 13 SECTION 1 GENERAL 1. Purpose: This procurement action establishes at least One (1) Indefinite Delivery/Indefinite Quantity (IDIQ) contract for the Department of Veteran Affairs, John J. Pershing VA Medical Center, 1500 Westwood Blvd, Poplar Bluff MO 63901, hereafter referred to as VA to provide supervised emergency housing, case management, transportation and meals for an estimated twenty-two (22) Veterans without dependents on any given day during the performance period to Veterans in the Poplar Bluff Missouri and surrounding areas. The period of performance for this contract will be for One (1) calendar year. No obligation will be incurred by the VA except for the services ordered and performed under this contract s terms and conditions to Veteran placements issued by the VA Homeless Program Staff for temporary emergency residential housing of placed Veterans. 1.1 The goal of temporary emergency residential housing is to remove Veterans from street inhabitation or other unfit habitations, and place them in community-based temporary housing that provides therapeutic services that meet the Veteran s needs, including those with mental issues, substance abuse issues, other medical/physical health issues and supports transition back into society. 1.1.2 The Contractor will not be required to provide detoxification or other hospital-level treatments. 1.2 Place of Performance (physical and mailing address): 1.1.3 Hours of Operation: The contractor will employ sufficient personnel to carry out the policies, responsibilities, including provision of transportation services when contractor-provided transportation is required, and the program for the facility. There must be sufficient administrative staff members, or designee of equivalent professional capability(ties), on duty on the premises at the facility and available for emergencies 24 hours a day, 7 days a week. The Contractor must at all times maintain an adequate workforce for the uninterrupted performance of all tasks defined within this PWS. The stability and continuity of the workforce staff shall remain sufficient as stated above while VA beneficiaries are receiving services. 1.3 Scope of Work: 1.3.1 Services, Non-Personal, the Contractor(s) shall furnish all resources (including, but not limited to facility(ies) (herein singular facility ), housing, lavatory and bathing area(s), furniture, equipment, materials and supplies, management, supervision, and labor) and services necessary to provide community-based emergency temporary residential housing accommodations, meal services, transportation services, case management services, and therapeutic and support services as specified herein to VA beneficiaries in accordance with the terms and conditions for beneficiaries who require services beginning on any day of the week, Sunday through Saturday and at variable hours on those days, and continuing for a period of time as authorized, in writing, by the authorizing VAMC. Services will be provided at the Contractor s facility, including transportation routes necessary for housing admission and appointments. The unit prices in the Price Schedule shall be inclusive of all services required by the Performance Work Statement. When services are required, the authorizing VAMC will initiate contact with the Contractor with pertinent information for each individual placement for entry into the residential facility. See SAMPLE Authorization Form. 1.3.2 The Contractor shall be capable of full performance of all required services on the effective date of contract award. Performance will begin on the effective date of contract award for any existing VA-placed Veteran on the effective date. Performance for each placement thereafter will begin on the date stated in the Emergency Housing Admission Form. 1.4 Recognized U.S. Federal Legal Holidays: Not applicable regarding contract performance since services are required every day of the 365-day period, except that U.S. Federal Legal Holidays are recognized by this contract and the U.S. Department of Labor (DoL) for compliance with Wage Determination Number 2015-5075, as issued and amended by DoL, in accordance with the Service Contract Labor Standards Act. The Wage Determination lists applicable U.S. Federal Legal Holidays and requirements for compliance. 1.5 The Government will monitor the Contractor s performance under this contract in accordance with the terms and conditions of the contract and the Quality Assurance Surveillance Plan (QASP). 1.5.1 The Contracting Officer and/or any other person designated in writing by the Contracting Officer will inspect the Contractor s facility to ensure compliance with federal, state and local health, sanitation, safety, security and fire prevention standards are being maintained, and that accommodations, meals and transports are being furnished in accordance with this PWS and resulting task orders or VAMC authorizations. Such inspections may be conducted at unannounced times. 1.5.2. The Contracting Officer and/or any person designated in writing by the Contracting Officer will monitor the Contractor s performance in accordance with this PWS and the Government s Quality Assurance Surveillance Plan. 1.5.2 The Contractor shall maintain a program of self-inspection, acceptable to the Government, to ensure compliance with this contract and its terms and conditions. Records of inspections shall be maintained and made available to the Contracting Officer or his/her designated representative when requested. 1.6 The Contractor and its employees shall be courteous to all persons served under this contract. Courtesy rendered shall be equal to that in commercial hospitality environments. SECTION 2 DEFINITIONS 2.1 ADA: Americans with Disabilities Act. 2.2 Agency: An executive department in the executive branch of the Government. 2.3 BDOC: Bed Day of Care (BDOC) is one (1) bed for 1 Veteran for 1 day, inclusive of all services herein. BDOC is the ordering, performance and billing Unit of Measure. 2.4 Case Management: The coordination of multiple community services to assist and guide clients toward achieving suitable housing, employment, social relationships, and community participation. 2.5 CFR: Code of Federal Regulations. 2.6 Contracting Officer (or CO): A person with the authority, within his/her limits of authority, to enter into, administer, and/or terminate the contract, and make changes to the contract. 2.7 Contracting Officer s Representative (or COR): A person, including a Contracting Officer s Technical Representative (COTR), designated and authorized in writing by the Contracting Officer to perform specific technical or administrative functions and is subject to strict limitations. 2.8 CPARS: Contractor Performance Assessment Reporting System. 2.9 Chronically homeless: As defined by 42 U.S.C. 11360(2)(A). 2.10 Community Integration: Community integration (CI) refers to a person s belonging and full participation in society includes three (3) principal areas: employment or other productive activity, independent living, and social activity. 2.11 FAR: Federal Acquisition Regulation. 2.12 HCHV: The Health Care for Homeless Veterans Program. 2.13 Homeless: As defined by 38 U.S.C. 2002(a)(1) as a Veteran who is homeless, and 42 U.S.C. 11302(a). 2.14 Invoice: Demand for payment for services performed. 2.15 NCO 15: Network Contracting Office 15. 2.16 NFPA: National Fire Protection Association, includes NFPA 10 (Standard for Portable Fire Extinguishers and NFPA 101 (Life Safety Code) 2.17 PWS: Performance Work Statement. 2.18 QASP: Quality Assurance Surveillance Plan. 2.19 Services, Non-Personal: The Government does not exercise any supervision or control over the contract service workers performing the services herein. Such contract service workers shall be accountable solely to the Contractor who, responsible to the Government. 2.20 USC: United States Code. 2.21 VA: Veterans Affairs. 2.22 VAMC: Veterans Affairs Medical Center. 2.23 VAAR: Veterans Affairs Acquisition Regulation, the agency supplement to the FAR. 2.24 Veteran: A Veteran is the VA-placed and/or referred VA Beneficiary, also referred as Veteran, VA-placed beneficiary, VA-beneficiary, Beneficiary or patient, or any combination of those terms, to receive services under this contract. 2.25 VHA: Veterans Health Administration. 2.26 VISN: Veterans Integrated Service Network. SECTION 3 GOVERNMENT-FURNISHED PROPERTY 3. None, except that Veterans VA-issued medications or other correspondences may be issued via U.S. Postal Service mail or carrier to the Veterans, care of the Contractor to the Contractor Address as coordinated by the authorizing VAMC. Contractor is required to provide distribution of mailed items, including non-VA correspondence and parcels, to the respective Veterans upon receipt. 3.1 Contractor mailing address: SECTION 4 CONTRACTOR-FURNISHED PROPERTY 4. The Contractor(s) shall furnish all resources (including, but not limited to facility, housing, lavatory and bathing area(s), furniture, equipment, materials and supplies, management, supervision, and labor) and services, food, meal services, transportation services, case management services, and therapeutic and support services, except that specifically identified as Government-furnished property in Section 3. SECTION 5 PERFORMANCE WORK STATEMENT 7 SUB-SECTIONS SUB-SECTION 5.1 LODGING 5.1 The Contractor shall provide each VA-placed Veteran day and overnight lodging (sleeping facilities, one individual per bed), including multiple days and nights, either back-to-back days and nights, or with breaks in between days and/or nights. Arrivals to the facility may be any day or night, such as day, evening, middle of the night or late night/early morning hours. See Sub-Section 5.6 regarding authorized absences (or passes ). 5.1.1 The Contractor shall enforce curfews for accurate daily Veteran resident census and Veteran tracking. See Sub-Section 5.6 regarding missing or absent residents and census. 5.1.2 Stay durations vary and depend on several factors, including but not limited to the number of days (or date range) required and stated on VA authorizations respective of each Veteran s needs or situation. Typical authorized periods are 30 days. Extensions may be issued from time to time. See SAMPLE Extension Form 5.1.3 Room and Board shall consist of a space for each Veteran including a clean and serviceable 1-person bed with mattress and pillow and appropriate linens, storage for clothes and personal items, and adequate towels and hand/wash cloths, and wake-up service. 5.1.4 The Government assumes no liability for any lodging damage caused by Veteran residents. 5.1.5. Environment shall be safe, secure and provide for privacy of the Veteran and be free of discriminatory behavior, including but not limited to free from discrimination based on age, disability, genetic information, national origin, race, color, religion, sex, and sexual orientation and any other not specifically listed here. 5.1.5.1 Environment shall be free of sexual harassment and any type of retaliation or retaliatory behavior. 5.1.5.2 Environment shall be conducive to social interaction and development of rehabilitation potential. 5.1.6 If facility houses other than adult male gender clients, the facility must have separate and secure lavatory and bathing arrangements to ensure privacy. 5.1.7 Secure storage for Veterans personal belongings is required, whether in the sleeping room or in a centralized location, secured at all times, and managed by the Contractor. 5.1.8 Laundry services shall be available to the Veteran. Services may be in the form of an adequate number of Veteran-operated self-service or Contractor-operated clothes washer and dryers - alternate means of laundering shall be available in the event of equipment outage. 5.1.8.3 Laundering processes shall ensure adequate extermination processes to remove infectious disease and infestations, killing any bacteria, rodents/insects and/or eggs/larvae that may exist accompanied with Veteran and Veteran belongings. The Contractor shall maintain an adequate protocol for processing clothing and belongings of new admissions and for continuous bacteria, rodent/insect and/or egg/larvae control. 5.1.8.4 For all new admissions, laundry is to be washed and dried immediately before entering the facility. 5.1.9 The Contractor shall exercise cleanliness in a manner like that of commercial hospitality environments. 5.1.10 The Contractor shall make available for Veteran resident usage telephone access and computer with internet access at Contractor cost, including hardware (e.g. computers, monitors, speakers, printers, etc.) and related software. 5.1.10.1 Residents shall be afforded access to receive and make local and toll-free telephone calls and conduct necessary business in conjunction with their stay to secure employment, housing, community outreach services and connection with family and social contacts. Access shall include privacy during their calls. The Contractor shall manage incoming telephone calls in such a way to protect the confidentiality of all Veterans receiving calls. 5.1.10.2 Residents shall be afforded access to computers with internet access. Where more than 1 computer with internet access is available, the internet service should allow for simultaneous access for multiple users. The service/speed of the internet service shall be commensurate with and sufficient for the number of simultaneous computers being used at any given time. The Contractor shall be responsible for establishing protocols concerning usage time/schedules and hours of availability. The Contractor shall be responsible for establishing any computer settings deemed appropriate, such as internet content-blocking measures. SUB-SECTION 5.2 MEALS 5.2 The Contractor shall provide scheduled nutritional meals (breakfast, lunch and dinner) and nutritional snacks between meals, and provide for unscheduled meals or snacks between scheduled meal times and bed time for those requiring or desiring additional food when not otherwise medically contraindicated. 5.2.1 Unscheduled meals may be in the form of sack lunches. 5.2.1.1 Unscheduled meals must be made available for Veterans arriving at the facility during non-standard workday hours. 5.2.1.2 Sack lunches are required for those who will not be present during normal scheduled meal times for absences such as medical appointments, employment, and community-outreach appointments. 5.2.1.2 The Contractor shall adhere to federal, state and local requirements for safe food storage, handling, preparation and safety, including, but not limited to having and maintaining necessary permits and licenses as applicable to the facility. Copies of current permits and licenses shall be provided to the Contracting Officer on demand. SUB-SECTION 5.3 TRANSPORTATION 5.3. Transportation: The Contractor shall assist the Veterans with transportation to scheduled meetings and appointments to the John J. Pershing VAMC and other VA schedule appointments within the city limits of Poplar Bluff MO. The Contractor will be expected to help the Veteran access public transportation, including providing information and instructions necessary to enable Veterans to utilize public transportation. If VA staff determines that adequate public transportation is not available or appropriate for a Veteran, the Contractor shall be responsible for the arrangement and payment of alternative transport as part of basic services. 5.3.1. The Contractor shall provide daily drop-offs and pick-ups at 1500 Westwood Blvd., Poplar Bluff, MO 63901, Monday through Friday at 7:00am, 12:30pm, and 4:00pm, including any VA scheduled appointments on Saturdays. In addition to the above, the Contractor shall be responsible for transporting Veterans: 5.3.1.1. (1) to the Contractor's facility, 5.3.1.2. (2) for initial admission to the program, 5.3.1.3. for Veterans/patients being discharged from VA, 5.3.1.4. for emergent housing needs from any of the seven Community Based Outpatient Clinics located at: 3051 William St., Cape Girardeau, MO 63701 heavier usage 1580 W. Columbia Dr., Farmington, MO 63640 heavier usage 903 South Kings Highway, Sikeston, MO 63801 heavier usage 1801 E. State Route K., West Plains, MO 65775 2420 Linwood Dr., Suite 3, Paragould, AR 72450 300 Camp Road, Pocahontas, AR 72455 35629 Highway 72, Physician s Office Bldg. 1, Suite 1, Salem MO, 65560 5.3.2. Transportation to the Contractor's facility will be provided in a timely manner upon receipt of authorization from the HCHV coordinator (i.e. COR), the AOD or other authorized VA staff. 5.3.3. All traffic laws shall be adhered to when transporting Veterans, to include operating in a safe, legal and courteous manner, rendering the same safety standards that would be afforded by any other chartered or public transportation service. 5.3.4. Drivers shall not engage in text messaging (send/receive/view) or other use of mobile phones, other than hands-free devices, while transporting Veterans. SUB-SECTION 5.4 CASE MANAGEMENT 5.4. The Contractor will employ sufficient professional personnel to carry out the policies, services, and responsibilities, at all times when VA-placements are residing at the facility, to interact with Veterans and provide services listed herein and to manage any issues or emergencies. 5.4.1. The Contractor shall ensure all persons performing the requirements of this contract have and maintain current Cardiopulmonary Resuscitation (CPR) and Basic Life Support (BLS) certification and that copies of certifications are available to the VA on request. 5.4.2 The Contractor Key Personnel list: See Attachment 2. Personnel other than clinical supervisor and any licensed Social Worker need not be listed except to satisfy 5.4.1.2. 5.4.3 Therapeutic and Rehabilitative Services: 5.4.3.1 Ensuring the Veteran s safety is high priority at all times. 5.4.3.2 If, for any reason, that the Contractor determines that the quality of care for a Veteran is at risk, or the Veterans may cause any danger of harming himself or others, the Contractor shall contact the HCHV Program Coordinator or designee by phone call within twenty-for (24) hours and written copy by fax within twenty-four (24) hours. The Contractor is encouraged to call the Veteran Crisis Line at 1(800) 273-8255 to assist with determination. If safety concerns persist or danger is immediate or imminent, local emergency services resources, such as the 9-1-1 emergency telephone number, should be activated. 5.4.3.3 Support for an alcohol and drug abuse-free lifestyle provided in an environment conducive to social interaction and the fullest development of the Veteran s rehabilitative potential. 5.4. The Contractor shall notify the authorizing VAMC facility immediately of any critical incidents involving Veterans placed and residing in the residential facility and make a written report of the incident. 5.4.4 The Contractor shall notify the HCHV Program Coordinator or designee by telephone during the hours of 8:00am and 4:30pm. for all incidents. For incidents that occur before or after normal business hours, the Contractor should notify the Administrative Officer of the Day (AOD). The Contractor shall provide the HCHV Program coordinator or designee with a copy of the incident report within 24 hours. The Contractor shall maintain a copy of the incident report in the Veteran's case record. 5.4.5 Critical Indents shall include but are not limited to: 5.4.5.1 Bodily injury(ies), such as falls, hits, cuts, burns, etc. 5.4.5.2 Elderly/dependent adult abuse or neglect 5.4.5.3 Sexual assault (Veteran involvement) (of any nature) 5.4.5.4 Fire (Veteran involvement) 5.4.5.5 Medical or mental health emergency (911 calls, or similar type) 5.4.5.6 Hospitalization 5.4.5.7 Suicidal ideations or attempts 5.4.5.8 Homicidal ideations 5.4.5.9 Assault (Veteran involvement) (any nature) 5.4.5.10 Death 5.4.5.11 Infectious disease concerns, infestations of rodents/bugs 5.4.5.12 Active substance abuse 5.4.5.13 Possessions of weapons (any type) 5.4.5.14 Police involvement/intervention 5.4.6 The Contractor shall notify the authorizing VA facility immediately when a medical or mental emergency occurs that requires hospitalization of Veteran. 5.4.6.1 The nearest appropriate VA hospital facility shall be utilized - except when the nature of the emergency is such that VA hospital utilization is not feasible, then hospitalization in a non-federal hospital may be accomplished when VA authorization is obtained. 5.4.6.2 Some emergencies may not allow for advanced VA authorizations. In those cases, notification to the HCHV Program Coordinator or designee within twenty-four (24) hours is required. 5.4.6.3 If hospitalization for non-emergent care is required, the nearest appropriate VA hospital facility shall be utilized. SUB-SECTION 5.5 RECORDS and MEDICATIONS 5.5 The Contractor shall create individual Veteran case records for each VA-placed Veteran within 3 days of Veteran admission. Records shall include necessary VA and next of kin (or other close relation as appropriate) contact information. Case records shall be made available only to the HCHV Program Coordinator or designee. 5.5.1 Case records shall be maintained in security and confidence, required by the Confidentiality of Alcohol and Drug Abuse Patient Records (42 CFR part 2) and the Confidentiality of Certain Medical Records (38 USC 7332). This applies to records containing health information pertaining to drug addiction, sickle cell anemia, alcoholism or alcohol abuse, or infection with human immunodeficiency virus. 5.5.2 The Contractor shall at all times maintain Medication Management, Monitoring and Individual Storage of Medications. 5.5.3 Further, the Contractor shall: 5.5.3.1 Provide secure storage area and controlled access (secure locked area) for medications that are brought into the residence and used by the Veteran. 5.5.3.2 Provide for the Veteran to take the medication without any assistance from staff when capable. 5.5.3.3 Ensure all prescribed medications and written prescriptions are returned to the Veterans residing at the contracted facility on their day of discharged. 5.5.3.4 Document up-to-date individual records of all medications, including prescription and non -prescription medications. 5.5.3.5 Develop and follow written protocols that address storage (including storage per manufacturer s recommendation) and handling of medications, safe disposal, and documentation of medication use, periodic checks for expiration dates or deterioration. 5.5.3.6 Provide information about resources for advocacy to assist Veterans in being actively involved in making decisions related to the use of medications, including providing information about training and education regarding medication. 5.5.3.7 Document showing that medications usage by Veterans is reviewed on at least twice a week during a residence period for each Veteran. 5.5.3.8 Maintain written protocols that address administration of medications by Contractor personnel, including staff credentials and competencies, documentation of medication administration, and documentation of the use and benefits, or lack thereof, of as needed doses. 5.5.3.9 Maintain written protocols regarding medications that provide for compliance with all applicable laws and regulations pertaining to medications and controlled substances. 5.5.3.10 Maintain documentation or confirmation of informed consent for each medication administered. 5.5.3.11 Maintain written protocols that integrate any prescribed medications into a person's overall plan, including, where applicable, special dietary needs and restrictions associated with medication use. 5.5.3.12 Maintain protocols that ensure the identification, documentation, and required reporting, including to the prescribing professionals, any medication reactions or medication errors, as appropriate 5.5.3.13 Maintain written protocols which include actions to follow in case of emergencies related to the use of medications, including ready access to the telephone number of a poison control center by both program personnel and participants 5.5.3.14 Evidence to the Contracting Officer that Contractor staff responsible for medications monitoring are familiar with all medications stocked including indications, side effects, toxic effects, interactions, and potential allergic reactions. 5.5.3.15 Monitor Medication quantities to prevent outages. 5.5.3.16 Refill and assisting refill of prescriptions using VA s automated system. SUB-SECTION 5.6 REFERRAL, ACCEPTANCE and ADMISSION CANCELLATION, ABSENCE and DISCHARGE 5.6 The Contractor shall accept VA referrals. If referrals cannot be accepted by the Contractor, the Contractor shall refuse, in writing, with sufficient information provided in the written refusal to support the refusal reasons. See 5.6.2 for limitations. 5.6.1 The VA, at its sole discretion, determines a Veteran s eligibility for referral to the Contractor facility. 5.6.1.1 The VA reserves the right to terminate authorizations and remove any or all Veterans from the facility at any time for any reason, without cost to the Government, when the VA determines removal is in the best interest of the Government and Veteran. 5.6.2 The Contractor shall not refuse or otherwise deny VA-referred Veterans entry to the temporary emergency residential housing facility based solely upon the length of current abstinence from alcohol or drugs (e.g. prescription or non-prescribed controlled substances or other illicit drug) or current alcohol or drug use, any prior treatment episodes, the time interval since the prior residence at the facility or other facilities, the current use of controlled substances, or legal history or any combination of these. 5.6.3 A written authorizing referral shall be provided to the Contractor for each Veteran referred for services under the contract. The written referral shall contain the Veteran s identifying information, the dates/duration of authorized placement, the name of authorized HCHV Program Coordinator or designee. The written referral is only valid when signed by the coordinator or designee. 5.6.3.1 The Contractor shall acknowledge the authorization by signature and return the signed copy by fax to the HCHV Program Coordinator or designee. 5.6.4 The Contractor shall provide reports of daily veteran occupancy/presence census on a weekly basis to the HCHV Coordinator or designee - Veteran sign-in/sign-out logs to verify placement attendance at the facility. Logs shall be provided with each monthly invoice, and a copy to the Contracting Officer upon request. 5.6.5 A list of HCHV Program Coordinator(s) and designees shall be provided to the Contractor at the time of contract award. The list may be updated periodically and by modification to the contract. 5.6.6 The initial referral period for a Veteran may be up to 30 continuous days, depending upon the needs of the Veteran and defined by the authorizing VAMC. Authorized referral periods may be reduced or extended, in writing, by the HCHV Program Coordinator or designee. 5.6.7 The Contractor shall notify the VA of any unauthorized absences of any VA-placed Veteran. 5.6.8 All requests for passes will be approved by the Homeless Coordinator or designees only and must be documented in writing in the Veterans individual case record. Please see SAMPLE Pass Form. 5.6.8.1 Authorized absences shall not exceed 48 hours in any given time month for any single Veteran, unless otherwise approved by the contractor and the HCHV Coordinator. 5.6.8.2 The Government will reimburse the Vendor for up to a maximum period of 1 day or 24 hours, provided there has been an active outreach attempt to relocate the Veteran and return the Veteran to the Vendor s facility. The Vendor shall provide an absentee report for validation of attempted search. Documentation will be added to the Veteran s case file. 5.6.9. Pass periods that include absence from the facility for full-day periods shall not be billed as BDOCs since BDOC services are not provided for that day. 5.6.10 Veterans may self-terminate (or self-discharge ) from the residential facility. 5.6.10.1 The Contractor shall notify the HCHV Program Coordinator or designee of any Veteran s notification of intent to self-termination discharge residence drop-out or walk-off immediately on notification of intent to self-termination discharge, or within 24 hours that such discharge type has occurred. The date of discharge must be approved by the HCHV Program Coordinator or designee for billing purposes. SUB-SECTION 5.7 CONDUCT and COMPLAINT MANAGEMENT 5.7 The Contractor personnel servicing this contract shall conduct themselves in a professional and courteous manner at all times when interacting placed Veterans, conveying attitudes of genuine concern and caring for the Veterans and their situations while also maintaining professional boundaries between staff and the Veterans. Veterans shall be treated with dignity and respect at all times. 5.7.1 The Contractor personnel shall under no circumstances engage in sexual activities or sexual contact with Veterans or their family members, whether such contact is consensual or forced. Contractor shall under no circumstances take unfair advantage of any professional relationship or exploit Veteran clients or their family members to further their personal, religious, political, or business interests. Contractor staff shall not engage in dual or multiple relationships with Veterans or their family members in which there is a risk of exploitation or potential harm to the Veteran or Veteran family. Contractor is responsible for taking steps to protect Veterans and their family members and is responsible for setting clear, appropriate, and culturally sensitive boundaries. 5.7.2 The Contractor shall be responsive to all Veteran complaints to ensure the requirements of this contract are satisfied without disruption. Should responsiveness require intervention by the VA, the HCHV Program Coordinator or designee should be contacted in sufficient time to alleviate the matter of the complaint. 5.7.3 The Contractor shall respond to VA-issued complaints in writing, including plans of correction where corrective action is required.
