Description

This amendment for solicitation # 36C26219Q0396 is to answer questions and extend the offer submission date. 1. Clarify the types and approximate quantities of Government Owned Cylinders that will be filled under this requirement. #8.1 on Page 2 of 2 in the SOW mentions filling Gov. Owned Cylinders but does not specify breadth or capacity. ANSWER: All government owned cylinders are annotated with a C/O at the end of the line item meaning Customer Owned. Therefore, offerors are instructed to not provide pricing for rent on customer owned cylinders. Line out the following line items: 6, 13, 22, 28, 30, 32, 34, 36, 40, 44, 46, 48, and 52. Offerors are also requested to offer pricing to refill government owned cylinders for all line items ending in C/O meaning Customer Owned. 2. Clarify the gas differences in the following Line Items as there appear to be multiple duplicate line items (perhaps to account for both facilities although this is not specified): a) 0003 & 0005 Both for Argon UHP Size 6K Line item 3 is contractor owned and line item 5 is to refill customer owned cylinders. b) 0007 & 0053 Both for Carbon Dioxide USP 50LB CGA 320 These are duplicates. Change line item 7 to 70 cylinders and delete line item 53. c) 0019 & 0021 Both for Nitrogen Medical NF 200 CGA 580 Line item 19 is contractor owned & line item 21 is to refill customer owned cylinders. d) 0025 & 0027 Both for USP Nitrous Oxide; Size 56LBs & Size 200 are the same cylinder These are duplicates. Change line item 25 to 155 cylinders and delete line item 27. e) 0031 & 0055 Both are for Medical Grade Oxygen Size 200, CGA 580 Line item 31 is to refill customer owned cylinders & line item 55 is for contractor owned cylinders. f) 0037 & 0039 Both are for medical grade Oxygen E Cylinders (Aluminum) Line item 37 is for contractor owned cylinders and line item 39 is to refill government owned cylinders. 3. Identify any outpatient clinics under this VA HCS that require direct delivery (there are six CBOCs listed on www.va.gov), specify any/all types of gases required, and delivery frequencies/schedules for each respective clinic. There are no CBOCs or VA clinics associated with this solicitation. Only the VA Loma Linda main hospital and ACC Redlands will place delivery orders. Additionally, all cylinders delivered to the VA Loma Linda Healthcare System are delivered to the loading dock. The loading dock is open from 6:00 am PST to 4:30 pm PST. Orders are placed Monday through Friday but deliveries are made Monday, Wednesday, or Friday. All orders placed or customer owned cylinders picked up are expected to be delivered by the following delivery day. For example, if an order is placed or a customer owned cylinder is picked up on Monday it needs to be delivered on Wednesday. If an order is placed or a customer owned cylinder is picked up on Wednesday it needs to be delivered on Friday. If an order is placed or a customer owned cylinder is picked up on Friday it needs to be delivered on Monday. The end user may modify this requirement based on the federal holiday schedule listed in the Statement of Work. Due to these changes, all offers must be emailed to bryan.boggs2@va.gov by 1200 noon PST on Monday, 2/25/2019.
