The requested diagnostic equipment is critical for audiology patient exams, hearing aid fittings / repairs / modifications / verifications and audiology C&P exams. Specifications as set forth in this proposal are minimum specifications and shall not be construed as limiting the overall quality, quantity, or performance characteristics of items furnished in the system. The Offeror shall meet or exceed the minimum requirements and shall be held responsible for the supply, performance and overall quality of the furnished equipment. All equipment to be supplied under the specifications shall be the newest and the most current model of a standard product of a manufacturer of record. A manufacturer of record is defined as a company whose main occupation is the manufacture for sale of the items of equipment and which: Maintains a factory production line. Maintains a stock of replacement parts for the item. Maintains engineering drawings, specifications, operating manuals and maintenance manuals. Has published and distributed descriptive literature and equipment specifications on the equipment. Operate both instruments from an optional touch panel for play audiometry, immittance testing, and patient counseling inside a sound booth. The software interface shall provide a user friendly and professional reporting tool. Also provide combined reporting including measurement data from both audiometry and immittance. INSTALLATION, INCLUDING VERIFICATION/VALIDATION, TRAINING: Technical support specialist must be able to assist in equipment installation/set-up, correlation studies (evaluation/comparison data sufficient to satisfy the audiology standards), staff training and methodology problems & questions. Vendor must provide all necessary procedure manuals, troubleshooting manuals and operator manuals (prefer computer disk/hard drive format). Vendor must provide education/training on-site and provide basic operating and maintenance training during installation as well as provide updates to the equipment when needed. Interface with VISTA/LIS Upgrades/ Test Volume Adjustment: Vendor shall work with the facility to keep the sound suite properly calibrated. Once testing is complete, the instrument sends the result back to the SNI which then sends the result back to VISTA in the appropriate format. All testing is performed by the Audiology Service staff using appropriate procedures, access and verify codes. Results are stored in the VISTA/LIS database and all records are backed up on disk and stored in a secure location, per regulations. Data will be available to providers using CPRS system. The above ppi will stay on the instrument s hard drive for about one month and then will be written over by new patient data. At the end f equipment, the hard drive and any other data storing devices will be removed from the instrument by VA and be taken to IRM for disposal at no cost. (End of Clause) C.3 VAAR 852.219-74 LIMITATIONS ON SUBCONTRACTING MONITORING AND COMPLIANCE (JUL 2018) (a) This solicitation includes VAAR 852.219-10 VA Notice of Total Service- Disabled Veteran-Owned Small Business Set-Aside. (b) Accordingly, any contract resulting from this solicitation is subject to the limitation on subcontracting requirements in 13 CFR 125.6, or the limitations on subcontracting requirements in the FAR clause, as applicable. The Contractor is advised that in performing contract administration functions, the Contracting Officer may use the services of a support contractor(s) retained by VA to assist in assessing the Contractor's compliance with the limitations on subcontracting or percentage of work performance requirements specified in the clause. To that end, the support contractor(s) may require access to Contractor's offices where the Contractor's business records or other proprietary data are retained and to review such business records regarding the Contractor's compliance with this requirement. (End of Clause) C.4 VAAR 852.232-72 ELECTRONIC SUBMISSION OF PAYMENT REQUESTS (NOV 2018) (a) Definitions. As used in this clause (1) Contract financing payment has the meaning given in FAR 32.001; (2) Designated agency office means the office designated by the purchase order, agreement, or contract to first receive and review invoices. This office can be contractually designated as the receiving entity. This office may be different from the office issuing the payment; (3) Electronic form means an automated system transmitting information electronically according to the accepted electronic data transmission methods and formats identified in paragraph (c) of this clause. Facsimile, email, and scanned documents are not acceptable electronic forms for submission of payment requests; (4) Invoice payment has the meaning given in FAR 32.001; and (5) Payment request means any request for contract financing payment or invoice payment submitted by the contractor under this contract.
