Page 1 of 3 COMBINED SYNOPSIS/SOLICITATION IR (IMAGED GUIDED) TABLE SYRACUSE VA MEDICAL CENTER This is a combined synopsis/solicitation for a commercial item prepared in accordance with the format in FAR Subpart 12.6 and 13.106, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a separate written solicitation will not be issued. This solicitation contains BRAND NAME specifications. Only brand name STERIS items will be considered as specified in paragraph VII. Only brand name STERIS items are requested in accordance with FAR SubPart 6.302-1. No other supply by any other manufacturer will meet agency requirements. (iii) This solicitation is issued as a request for quote (RFQ). Request for Quote will be submitted through fedbizopps.gov on RFQ reference number 36C24219Q0379. (iv) This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-95. (v) This procurement is being issued as 100% set-aside for SERVICE DISABLED VETERAN OWNED SMALL BUSINESSES (SDVOSB). It is the Government s intent to issue a firm fixed price, single award as a result of this solicitation. The North American Industry Classification System (NAICS) code is 339113, Surgical Appliance Supplies Manufacturing, with a small business size standard of 1250 employees. This solicitation is 100% set-aside for SDVOSBs. (vi) The Contractor shall provide all parts necessary to deliver STERIS brand name as specified below. The requested products shall be delivered in new and unused/unopened condition. (vii) Description of requirement: The contractor shall provide as manufactured by STERIS: Item # Part # Description Quantity 1 TA-001 SurgiGraphic 6000 Image Guided Surgical Table Image guided table used for minimally invasive vascular, endovascular, and pain management procedures. Comprised of a carbon fiber tabletop cantilevered from a single column and mobile base assembly. Top includes lateral and longitudinal manual four-way float motion and a unique edge contour that facilitates accessory attachment while enabling 360? patient imaging capability with a fluoroscopic c-arm. Offers a 500 lb. weight capacity. 1 2 TA-005 SURGIGRAPHIC STANDARD ACCESSORY PACKAGE 1 3 SE1100300072 Inspect and Start-Up OT1000/SG6000 Image Guided Table STERIS Service will stage the table in the location designated by the Customer for review and check-in by the facility. STERIS will: Uncrate the table/re-crate trade-in (if applies); Ensure that power is fully charged for an all-systems check; Fully inspect table condition and supporting hardware; Perform and complete STERIS 31 point start up test to confirm optimal table performance; Verify floor locking system is secure; Confirm tilt speed; Inspect shroud to ensure complete functionality and verify no damage during freight transportation. Remove and properly dispose of all crating material. 1 4 SE115031 Warranty Performance Check Surgigraphic 6000 Table This service is included free of charge during the warranty period; the value of this Is $225. The Warranty Performance Check inspection supplements the standard STERIS warranty and is scheduled at the 7th month. The local Field Service Rep will use the Preventive Maintenance Checklist to inspect and test the equipment, and will make any necessary adjustments or repairs 1 Only offers meeting the above specifications shall be considered. RFQ Responses should consider/include any shipping costs into quotes. Shipping will not be paid for as a separately billable line item. (viii) Awardee shall coordinate with the Official Mail Center (OMC) prior to performance.
