Page 2 of 21 SUBJECT: Flooring Installation at the Baltimore VAMC Facility DISCLAIMER: This Sources Sought is issued solely for information and planning purposes and does not constitute a solicitation. Neither unsolicited proposals nor any other kind of offers will be considered in response to this Sources Sought. Responses to this notice are not offers and will not be accepted by the Government to form a binding contract. Responders are solely responsible for all expenses associated with responding to this Sources Sought. All information received in response to this Sources Sought that is marked Proprietary will be handled accordingly. Responses to the Sources Sought will not be returned. At this time, questions concerning the composition and requirements for a future RFP will not be entertained. SUBJECT: The Baltimore VA Medical Center, is conducting market research to provide new flooring and installations throughout the facility. OBJECTIVE: See Attachment 1 WORK SPACE DRAWINGS: See Attachment 2 RESPONSE INSTRUCTIONS: Name and Address of Company Any small business designation as certified by the Small Business Administration A through description of experience in managing support requirements of this nature. Responses due by 02-28-2019 @ 3:00PM CONTACT INFORMATION: Any questions related to this RFI shall be directed to Tyler Hallock (robert.hallock@va.gov). All information regarding Capabilities Statements or any other proprietary information relative to this RFI shall be submitted via email to robert.hallock@va.gov no later than 3:00 pm EST on 02-28-2019. Attachment 1: OBJECTIVES GENERAL REQUIREMENTS: PURPOSE: To supply and install various types of flooring in misc. areas in the VA Maryland Healthcare System to include but not limited to, Baltimore and Loch Raven as designated by the Interior Design Department. Site visit information will be provided and the contractor is strongly encouraged to participate as the government will not be responsible for overages or underestimates in flooring amounts. This is just a sources sought but upon issuance of the RFQ, we will provide site visit dates that will range from Monday through Friday. WORK ADDRESS: Baltimore VA Medical Center 10 N. Greene St. Baltimore, MD 21201 GOVERNMENT REQUIREMENT: The installations must be performed by a certified installer. The contractor shall designate an on-site working supervisor. They will coordinate and be responsible for directing the crew members. Floor plans of the space will be provided by Interior Design (Attachment 2). The vendor is responsible for measuring the space for final requirements and to determine what needs to be done for removal of the old flooring. (See site visit information) Any abatement issues will be taken care of by the VA. The contractor shall provide all the tools, labor, materials parts, finishes and equipment necessary to receive, deliver and install the flooring. Adequate access will be provided to the building while performing the work. During work if site conditions differ from the site survey, a written change proposal will be provided describing how conditions differ from the SOW and what impact it may have on the progress of work and how it will affect the cost of work. Installers are responsible for any damage done to VA popery while on site. They should contact the Interior Designer immediately so it can be documented and the correct repair be done by the installer/contractor. The contractor is responsible for removing all discarded paper, boxes and trash they create from the site. There will be no smoking, eating or drinking inside the building. Smoking is allowed in designated areas outside the building only. After completion of the installation, a walk through with the installer and the Interior Designer will be done to check for deficiencies. The contractor will complete any wrong findings or damages and will be corrected by the dealer at no additional cost to the government. The C & A requirements do not apply. A Security Accreditation Package is not required. QUALITY ASSURANCE: Installer Qualifications: A company specializing in installation with minimum three (3) years experience and employs experienced flooring installers on commercial applications. We prefer experienced Installers who have retained, and currently hold, an INSTALL Certification, or a certification from a comparable certification program but this is not a requirement. 1. Installers to be certified by INSTALL or a comparable certification program with the following minimum criteria: 2. US Department of Labor approved four (4) year apprenticeship program, 160 hours a year. 3. Career long training. 4. Manufacturer endorsed training. 5. Fundamental journeyman skills certification. If you do not hold a certification please provide a list of other flooring contracts that we can review to insure we pick the right contractor for the job. STANDARDS: Work to be performed on the weekends between the hours of 8:00am-4:30pm, or other hours as scheduled by the Interior Designer. REQUIRED REPORTING: Contractor personnel are required to check in with the chief of the using service prior to starting work and check out at the end of the work day. Chemicals: Contractor will furnish a list to the Interior Designer prior to performance of any work, the trade names (if any) and chemical names and strengths used along with any antidotes that are used in the performance of the services. All vendors are subject to Federal Installation Security Requirements, including inspection of any containers, equipment or vehicles as required. SPECIAL PROVISIONS: All rooms need to be properly isolated with zip walls, cover all vents, hard exterior barrier and a negative air machine as required by Infection control. An Infection Control Construction Permit provided by the VA, must be posted. LOCATION: VA Medical Center Healthcare System: Location 1. 10 N.
