Description
This is a SPECIAL NOTICE ANNOUNCEMENT ONLY.   It is neither a solicitation announcement nor a request for proposals or quotes and does not obligate the Government to award a contract.    Requests for a solicitation will not receive a response. Responses to this Sources Sought must be in writing.   The purpose of this Special Notice Announcement is for market research to make appropriate acquisition decisions and to gain knowledge of potential qualified Service Disabled Veteran Owned Small Businesses, Veteran Owned Small Businesses, 8(a), HubZone and other Small Businesses interested and capable of performing the work. Documentation of technical expertise must be presented in sufficient detail for the Government to determine that your company possesses the necessary functional area expertise and experience to compete for this acquisition.      Responses to this notice shall include the following: (a) company name b) address (c) point of contact (d) phone, fax, and email (e) DUNS number (f) Cage Code (g) Tax ID Number (h) Type of small business, e.g. Services Disabled Veteran Owned small Business, Veteran-owned small business, 8(a), HUBZone, Women Owned Small Business, Small disadvantaged business, or Small Business HUBZone business and (i) must provide a capability statement that addresses the organizations qualifications and ability to perform as a contractor for the work described below. Statement of Work Background: Cleveland Clinic is a nonprofit, multispecialty academic medical center that integrates clinical and hospital care with research and education. Among Cleveland Clinic s 51,000 employees are more than 3,500 full-time salaried physicians and researchers and 14,000 nurses, representing 140 medical specialties and subspecialties. Cleveland Clinic s health system includes a 165-acre main campus near downtown Cleveland, 10 regional hospitals, more than 150 northern Ohio outpatient locations including 18 full-service family health centers and three health and wellness centers and locations in Weston, Fla.; Las Vegas, Nev.; Toronto, Canada; Abu Dhabi, UAE; and London, England. In 2016, there were 7.1 million outpatient visits, 161,674 hospital admissions and 207,610 surgical cases throughout Cleveland Clinic s health system. Patients came for treatment from every state and 185 countries. U.S. News & World Report consistently names Cleveland Clinic as one of the nation s best hospitals in its annual America s Best Hospitals survey. Communicate with H.E.A.R.T. ® is the standard for teaching staff to communicate with empathy. It was developed by healthcare professionals, this innovative program will help VA Hudson Valley employees understand their role in creating positive patient experiences associated with their job. Using interactive training and easy-to-remember tools, Communicate with H.E.A.R.T. ® will empower staff to provide an exceptional patient and employee experience at every point of interaction. Objectives: For Hudson Valley Health Care System to become the health care system of choice for Veterans in our catchment area and that Hudson Valley staff look forward to come to work. Although this is already a reality for many of our Veterans and staff, the training provided from Contractor will support VA Hudson Valley s continued efforts to advance patient-centered care, and strengthen Veteran and caregiver satisfaction. Period of Performance: The performance period for SOW will be a period of one calendar year from the date of the award. If the contractor proposes an earlier completion date, and the government accepts the contractor s proposal, the contractor s proposed completion date shall prevail. Work at any government site shall not take place on Federal holidays or weekends unless directed by the CO. Place of Performance: Work will be performed on-site at VA Hudson Valley Healthcare System. Scope of Work: Cleveland Clinic will provide customized training on site during their two-day site visit. Hudson Valley VA facilitators will participate in a Train the Trainer session where they will have the opportunity to learn new skills and practice facilitating the program while receiving constructive feedback from the Cleveland Clinic training team. Course Objective Time S.T.A.R.T. with Heart ® Expected Service Behaviors 2 Hours Respond with H.E.A.R.T. ® Service Recovery Model 2 Hours Train the Trainer Training Skills 8 Hours Type of Contract: Firm fixed price. Deliverables Schedule: Service Description Communicate with H.E.A.R.T. Program License Includes use of all program materials and a perpetual license that allows lifetime use of program. Delivery Fee 2-Day, onsite program delivery conducted by Cleveland Clinic training team Travel Expenses Travel expenses for Cleveland Clinic training team (actual expenses only) Customization First 8 hours* of collaborative customization services Government Furnished Property, Material, Equipment, or Information (GFP, GFM, GFE, or GFI): N/A Security: None. Contractor consultants will be escorted to work rooms by Hudson Valley staff. Contractor consultants will not need a key to any part of the facility nor will they be accessing any medical records. Safety Codes/Certification/Licensing: N/A Travel: The cost of travel to and from the VA Hudson Valley site is not part of Contractor s daily rate. Travel costs are estimated above and included in the total estimated costs. Special Material Requirements: None Any Other Requirements Unique to the Solicitation: None Applicable Standards: For each core element, Planetree has developed specific criteria that must be met in order to demonstrate that a site has implemented that aspect of patient- or resident-centered care. Designed to provide a level of consistency in what it means to be patient- or resident-centered while still continuing to promote individuality and innovation in the delivery of care, the criteria were developed with extensive input from Planetree affiliates and outside experts, including a Continuing Care Advisory Council and a Behavioral Health Advisory Council. The criteria are designed to be applicable to all healthcare providers, irrespective of size, location or formal affiliation with Planetree; however, the scope and breadth of these criteria reflect the experiences of sites that have been engaged in an ongoing, focused effort to cultivate a culture of patient- or resident-centered care over a period of several years. Important Information: The Government is not obligated to nor will it pay for or reimburse any costs associated with responding to this Special Notice Announcement request. This notice shall not be construed as a commitment by the Government to issue a solicitation or ultimately award a contract, nor does it restrict the Government to a particular acquisition approach.  The Government will in no way be bound to this information if any solicitation is issued. Currently a total set-aside for Small Business firms is anticipated. However, if response by Service Disabled Veteran Owned Small Business firms proves inadequate, an alternate set-aside or full and open competition may be determined. No sub-contracting opportunity is anticipated. The North American Classification System (NAICS) code for this acquisition is 611430 (Professional & Management Development Training). Notice to Potential Offerors: All Offerors who provide goods or services to the United States Federal Government must be registered in the System for Award Management (SAM located on the web at www.sam.gov).  It is desirable that any Offeror to have completed their business Online Representations and Certifications Application (ORCA). Additionally, all Service Disabled Veteran Owned Businesses or Veteran Owned Businesses who respond to a solicitation on this project must be registered with the Department of Veterans Affairs Center for Veterans Enterprise Vet-Biz Registry located at http://vip.vetbiz.gov.  All interested Offerors should submit information by email to Clifford.Harrison@va.gov and  should be received no later than 12:00 pm EST on February 28, 2019.
