Page 1 of 1 The Department of Veterans Affairs (VA) and the Iowa City Veterans Healthcare System (ICVHS), in Iowa City, Iowa, are issuing a Request for Proposal (RFP) for Remote Cardiac Outpatient Telemetry and Wireless Cardiac Event monitoring services. The VA anticipates one fixed-price/IDIQ contract award. This is a COMBINED SYNOPSIS/SOLICITATION notice for commercial services prepared in accordance with VAAR Part 873, supplemented with additional information included in this notice. This announcement constitutes the only solicitation. This solicitation is a 100% set aside for Women-Owned Small Businesses (WOSB). The due date for proposals is 12 March 2019 at 12:00 PM CST. The North American Industry Classification System (NAICS) code assigned to this procurement is 621999 All Other Miscellaneous Ambulatory Health Care Services, with a size standard of $15 million. Offerors will be evaluated on technical capability (equipment capabilities and general requirements; management; operation plan), past performance, and price evaluation. The solicitation anticipates requirements for five years. Service will begin approximately 01 April 2019 (or date of award) through 31 March 2024. The Offeror is responsible for downloading all documents including amendments without further notification from the VA Contracting Officer. The RFP must be completed in its entirety and included in the response. Proposals must be submitted in writing to the issuing office, be in accordance with the preparation instructions outlined in the solicitation and be timely. It is the responsibility of the Offeror to ensure their proposal is received in full by the Contracting Office before the solicitation closing. Late proposals will not be accepted. Contractors must be registered in the System for Award Management (SAM). Lack of registration in the System Award Management (SAM) database (www.sam.gov) will make a vendor ineligible for award. https://www.sam.gov/ This solicitation incorporates provisions and clauses in effect through FAR, VAAR and Federal Acquisition Circular 2005-99.
