- 1. INTRODUCTION: This sources sought notice is for information and planning purposes only and shall not be construed as a solicitation or as an obligation on the part of the Department of Veterans Affairs (VA). The VA is currently conducting market research to locate qualified, experienced and interested potential sources. Responses will not be considered as proposals, nor will any award be incurred by interested parties in responding to this sources sought announcement. NCO 1 Contracting is seeking interested sources for a future solicitation and procurement. At this time, no solicitation exists. 2. BACKGROUND: VA Community Living Centers are surveyed annually by the Long-Term Care Institute (LTCI) to determine the level of compliance with CMS Conditions of Participation (CoPs). The New England VA Healthcare network is seeking non-personal services to provide Centers for Medicare & Medicaid Services (CMS) Advisory Services to VA Medical Centers within Veterans Integrated Service Network (VISN) 1. Required services include: Support continual readiness by conducting a mock survey that assesses readiness and identifies CMS CoP risk areas. Provide a post mock survey analysis delivered to VISN-1 leadership to address all areas of compliance, deficiency, and strategies for improvement with an implementation plan for CMS compliance. Provide onsite support during the LTCI Annual unannounced survey Provide remote support during the LTCI Annual unannounced survey Provide post CMS survey review of deficiencies, development of a Plan of Correction, provide an assessment of an implementation plan, provide guidance on submission of plan to CMS. Provide CMS survey preparation training and practical exercises to educate VISN-1 personnel on Medicare survey requirements and survey processes. 3. OBJECTIVES: The VISN 1 healthcare system is looking for small business concerns, especially Service Disabled Veteran Owned Small Business (SDVOSB) and Veteran Owned Small Business (VOSB) companies that are experienced and capable of providing CMS audit and consulting services. 5 years of survey experience in the areas of acute care, long term care, and rehabilitation is preferred. All employees providing audit and consulting services must have a minimum of a bachelor s degree and have expert knowledge of all the federal conditions of participation/condition of coverage and report out using the Principles of Documentation. The contractor must have the ability to travel within 24-hours to assist clients in Immediate Jeopardy situations. The current primary NAICS code is 541611 (Medical office management consulting services or consultants), which has a size standard of 15 Million. 4. SUBMITTAL INFORMATION: Contractors having the skills and capabilities necessary to perform the stated requirements should submit a response as detailed below. Companies should provide clear and unambiguous evidence to substantiate their capability to fulfill the requirements above. Interested parties shall furnish the following minimum information: 1. Company name, address and DUNS number ensure your company is registered in the System for Award Management (SAM) at www.sam.gov. 2. Company point of contact name, telephone number, and email address. 3. Evidence that you are a small business (if applicable): Small Business ensure you are listed as small business in SAM, reps and certifications. Provide the NAICS codes you are listed as a small business under. CVE certified SDVOSB or VOSB (or in the process of certification) ensure you are certified in the CVE veteran database at www.vetbiz.gov. Provide evidence of certification. 4. If your company holds a Federal Supply Schedule (FSS) Contract, list the GSA Contract Number and SINs. 5. Capabilities/Qualifications: A written response providing clear and unambiguous evidence to substantiate the capacity to fulfill this requirement. Description of the capabilities/qualifications/skills your company possesses to perform services described above. 6. Past Experience: Brief summary of the company history relative to similar requirements. Offer may provide the following information on a maximum of three similar projects for which the responder was a prime or subcontractor.
