This announcement constitutes the only solicitation; quotes are being requested, and a written solicitation document will not be issued. This is a Request for Quote (RFQ). The government anticipates awarded a firm-fixed price contract resulting from this solicitation. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-101. The North American Industrial Classification System (NAICS) code for this procurement is 238310 and the small business size standard is $15M. This solicitation will be posted using Tiered Evaluation (Cascading) in accordance with the VA Rule of Two in accordance VA Procurement Policy Memorandum (PPM) 2018-04. TIERED EVALUATIONS The Network Contracting Office (NCO) 10-SAO Central is soliciting offers using Tiered Evaluations (Cascading) procedures from Service Disabled Veteran Owned Small Businesses (SDVOSBs), Veteran Owned Small Businesses (VOSBs), Small Businesses (SBs) to provide all management, supervision, labor, materials, supplies, tools and equipment, necessary to perform retherm replacement at the Louis Stokes Cleveland VA Medical Center. Respondents to this solicitation must fully demonstrate their capability by supplying detailed quote information along with any other documents necessary to support the requirements below. This solicitation is being issued as a tiered evaluation for SDVOSB concerns, or in the alternative, a tiered evaluation for VOSB concerns, or in the alternative, a tiered evaluation for other small business concerns with HUBZone small business concerns and 8(a) participants having priority, or in the alternative. If award cannot be made, the solicitation will be cancelled and the requirement resolicited. All offerors must be registered in System for Award Management (SAM) at http://www.sam.gov to be considered for an award of a federal contract registered in the record must be complete and not missing elements such as representations and certifications. Also, if you want consideration as a Service Disabled Veteran Owned Business (SDVOSB) or Veteran Owned Business (VOSB), you must be registered in VIP (Vendor Information Pages) at https://www.vip.vetbiz.gov/, if not, then you will be placed into the small business tier. All questions regarding this RFQ shall be emailed to Harvey J. McGowan, Contracting Officer at harvey.mcgowan@va.gov and submitted no later than 4:00 PM EST on March 5, 2019. Contractors interested in successfully completing this requirement shall submit their quotes via email to harvey.mcgowan@va.gov. All quotes are due by 4:00 P.M. Eastern Standard Time (EST) on March 8, 2019. Quotes received after this date and time will be considered late in accordance with 52.212-1(f) and will not be evaluated. Description of Services: The VHA NCO 1O Louis Stokes Cleveland VA Medical Center is seeking the following services: Contract Title. Retherm Replacement Background. New Retherms required new chilled water piping which is being completed in house by Engineering s second shift plumbing shop. This new chilled water piping needs to be insulated by insulation contractor to prevent dripping and condensation of exposed piping. Scope. Insulate all new chilled water copper piping in main kitchen with 1 fiberglass wall pipe insulation. Pipe fittings such as 90 s and Tees shall be Zeston or equal and taped for a proper vapor barrier. Insulate 6-FT washing machine hoses with removable ¾ wall flexible Armor flex insulation, field verify. Exposed fiberglass pipe insulation, valves, strainers and circuit setters shall be covered with a plastic outer barrier for housekeeping purposes. Exposed plastic outer barrier shall be rated for a food preparation area. Exposed Armor flex shall have an outer plastic or painted sealed rated for food preparation area. Specific Tasks. 1. Insulate chilled water mains, branch piping and all pipe accessories with 1 fiberglass wall insulation 2. Please oversize fiberglass insulation to properly cover all valves, strainers, circuit setters and any oversized pipe accessory 3. Approximate pipe and fitting insulation coverage (please field verify): 320 ft of 1 copper pipe 80 ea 1 copper 90 deg fitting 32 ea 1 copper tee fitting 4 ea 1 ball valves 160 ft ¾ copper pipe 32 ea ¾ copper 90 deg fitting 16 ea ¾ brass strainer 36 ea ¾ call valves 17 ½ circuit setters 32 ea 6ft washing machine hoses 32 ea 1 x ¾ x ¾ copper reducing tee Performance Monitoring. COR and Contractor will monitor performance. Security Requirements. Insulation contractor will be given access to the work area Monday thru Friday 7:00-pm thru 3:00-AM until completion. Government-Furnished Equipment (GFE)/Government-Furnished Information (GFI). There will be no government furnished equipment provided for the purposes of this contract Place of Performance. Wade Park VA facility at 10701 East Blvd Cleveland Ohio 44106 Period of Performance. From date of award until completion of service. Deliverables: All parts, labor, travel and freight will be included under the contract Contract Type: The government anticipates awarding a Firm Fixed Price Purchase Order/Task Order. Award will be made to lowest price quote which meet or exceed the requirements within this solicitation as demonstrated in the technical statement. Technical Statement. (a) Demonstrating relevant experience, skills and ability to fulfill the Government's requirements for the above. The quote in its technical statement should contain enough sufficient detail for the Government to make an informed decision regarding your capabilities; however, the statement should not exceed 10 pages. (b) The technical statement must identify the responder's business type and size. (c) The technical statement must provide company name and address, point of contact, phone/fax/email and NAICS Code(s). Confidentiality of Information In accordance with FAR 52.227-14(b), the contractor shall guarantee strict confidentiality of the information/data that it is provided by the Government during the performance of the contract. The Government has determined that the information/data that the Contractor will be provided during the performance of the contract is of a sensitive nature and cannot be disclosed in any manner. The Contractor, in whole or in part, can only make disclosure of the information/data, after the Contractor receives prior written approval from the Contracting Officer. Whenever the Contractor is uncertain about the proper handling of information/data under the contract, the Contractor shall obtain a written determination from the Contracting Officer. PAYMENT: CONTRACTOR REMITTANCE ADDRESS: Effective January 1, 1999 all payments by the Government to the contractor will be made in accordance with Federal Acquisition Regulation (FAR) Clause 52.232-34, Payment by Electronic Funds Transfer-- Other than Central Contractor Registration (31 U.S.C. 3332). Payments will be made monthly in arrears upon receipt of properly prepared invoices which must show reference to the correct purchase order number issued upon contract award.
