- 03/04/2019 THIS IS A SOURCES SOUGHT NOTICE ONLY. THIS IS NOT A REQUEST FOR A PROPOSAL OR QUOTE. THE VA IS NOT SEEKING PRICING OR OFFERS. This is a Request for Information (RFI)/Sources Sought Notice, issued in accordance with FAR 15.201(e), to conduct market research. This RFI/Sources Sought Notice is issued solely for information and planning purposes. It does not constitute either a Request for Quotation (RFQ) or a Request for Proposal (RFP); nor is it a promise to issue a RFQ/RFP in the future. This RFI/Sources Sought Notice does not commit the Government to contract for any supply or service whatsoever. The Department of Veterans Affairs (VA) is not seeking proposals at this time and will not accept any unsolicited proposals. Responders, and not the Government, will pay for any costs incurred while responding to this RFI/Sources Sought Notice. Vendors who don t respond to this RFI/Sources Sought Notice may still participate in any future RFQ/RFP. Information submitted in response to this RFI/Sources Sought Notice is strictly voluntary and becomes the property of the Government. The Government may modify this announcement in the future, as information changes. The Government is not soliciting offers at this time. This is a RFI/Sources Sought Notice only, to gain knowledge of potential qualified sources. A solicitation is not currently available. Responses to this request will be considered for the purpose of determining how the requirement will be stated and how the procurement will be conducted. This RFI/Sources Sought Notice is not to be construed as a commitment by the Government, implied or otherwise, to issue a solicitation or to award a contract. No reimbursement will be made for any costs associated with providing information in response to this synopsis or any follow-up information requests. This RFI/Sources Sought Notice is market research to help the Department of Veterans Affairs make acquisition decisions. The VA Heartland Network Contracting Office (NCO) 15 is looking for Small Business concerns, and VETBIZ VIP verified SDVOSB and VOSB concerns, all registered under the Product Service Code (PSC) of R499 (Support - Professional: Other) and NAICS code of 624190 (Other Individual and Family Services), in SAM, that are capable of performing this service as follows: Location of Service: Marion VA Medical Center 2401 West Main Street Marion, IL 62959-1188 Description of Services: Scope: The Contractor shall provide an indefinite delivery/indefinite quantity requirements contract for providing employee assistance services for employees of the Marion Veterans Affairs Health Care System (MVAHCS). Services include providing employee assistance in dealing with life at work and at home, provide short term counseling, problem identification assistance, problem resolution assistance, family counseling and will maintain confidentiality of employees who request these services. Dependents and family members that meet the dependency and familial relationships that are recognized by the Internal Revenue Service (IRS) are eligible for family counseling. Services provided to Marion Veteran Affairs Health Care System (hereinafter referred to as MVAHCS) shall include: Communication in the hospital for employees such as posters, orientation meetings, benefits fairs, including promotional in-services. Initial assessment and referral for: Psychological counseling Relationship counseling Marital and family counseling Alcohol and substance abuse counseling Stress, job or personal The contractor shall be physically accessible in local and outlying areas. Appointment times should be during normal business hours and some evening hours. Crisis accessibility shall be twenty-four hours a day, including weekends and holidays. The contractor shall respond to non-emergency requests within three business days. In emergency situations, the contractor shall provide same-day appointments. The contractor's staff shall have a minimum of a Master's degree in a Behavioral Health field and previous experience in a wide range of personal problems such as alcohol and drug counseling, marital and family related issues, and experience/expertise in resolving workplace problems and delivery employee assistance and related managed care services. The contractor shall furnish proof of current liability insurance in an amount not less than $1,000,000 per occurrence. The contractor shall provide the Contracting Officer s Technical Representative (COTR) with quarterly reports on usage trend for management that may help identify possible problem areas with MVAHCS. No information shall be provided to any person/persons, agency/agencies, in or out of the Federal Government which might identify an employee or a member of his/her family without a voluntarily signed release of information by the employee or his/her family member (or legal guardian if a minor) specifying to whom, and for what reason, the information is to be released. The Contractor shall establish a confidential record-keeping system which is in compliance with Federal standards, and with requirements relating to the confidential nature of patients records in accordance with Section 333, Public Law 91-616 (as amended by Public Law 92-282), Section 406 of Public Law 92-255, and Federal regulations of the Special Action Office for Drug Abuse Prevention (21 CFR 1401). General Requirements: Evidence of Insurance Coverage. Before commencing work under this contract, the contractor shall furnish to the Contracting Officer, a certificate of insurance indicating the coverage outlined and containing an endorsement to the effect that cancellation, of or any material change in the policies which adversely affect the interests of the Government in such insurance shall not be effective unless a 30-day advance written notice of cancellation of change is furnished the contracting officer. A monthly report shall be submitted to the Contracting Officer and Contracting Officer s Technical Representative detailing all calls received, when they were received, emergency or non-emergency and contractor response. The report shall be sanitized so that no personal information is revealed to protect the privacy of the patient.
