Description
DIETETICS BUILDING 300, SEISMIC REPLACEMENT DESIGN BUILD PROJECT 691-406 GREATER LOS ANGELES HEALTHECARE SYSTEM VA MEDICAL CENTER WEST LOS ANGELES, CA STATEMENT OF WORK The project consists of the disconnection and storage of an existing ground mounted solar farm, the construction of a new approximately 35,000 square foot multistory dietetics building per the VA Technical Information Library criteria to include but not limited to the VA Mission Critical Design and VA Seismic Design Requirements Manuals. The building will serve approximately 1,000,000 meals per year, consisting of quick chill meals until the new hospital is constructed. After activation of the new hospital, the kitchen will serve room service type meals to the hospital. The room service will be delivered to the hospital via an automated guide vehicle system via a tunnel which must be integrated into the base design of the new hospital. The contractor is expected provide; all design services, furnish labor, supervision, equipment, vehicles, a third party certificate of occupancy, and labor to construct a dietetics building per the bridging document. The contractor is expected to have visited the site to understand the traffic and delivery limitations. The work includes but is not limited to the following: Using the bridging documents, developed by VA s AE and provided by VA, prepare a final set of drawings for the VA review and approval prior to construction. The contractor will be expected to provide the Medical Center Leadership a presentation on the Construction Drawings for approval. Also to travel to Washington DC to present the Construction Drawings to the Consulting Support Service for their concurrence regarding the design. Construction Drawings must conform to VA Standards per the Technical Information Library website and current California Building Codes. Where a conflict exists, the strictest interpretation will apply. Drawings must be wet stamped by an architect, structural engineer and geotechnical engineer registered in the State of California. The Work includes, but not limited to the following: Excavation, paving, construction, secure material storage, site preparation, demolition, removal of existing structures, removal and installation of photo voltaic panels, disposal or storage of removed materials, permits, site security, safety plan. The contractor shall follow a strict staging plan to ensure a successful Project given the location and layout of the building and work, the interference with the pedestrian and vehicle traffic and fire lanes, noise generated on site, impact to adjacent properties such as dust. To minimize disruption to the adjacent property and to complete the Project on schedule, certain arrangement and construction sequencing need to be carefully thought out by the Contractor and approved by the VA, including efficient traffic routing. Notify the city, appropriate governmental agencies, and adjacent property owners prior to the Start of Construction, and a minimum of (5) five working days prior to performing any work which necessitates closing or creating limited access to park walkways, parking, fire lanes, driveways, and roads. Contractor will obtain and pay for any services of City Agencies or Permits required for Street Use/Traffic Control out of his Construction Contract. Contractor will obtain written permission from the City prior to effecting such temporary closures or interruptions. Contractor shall coordinate with VA and governmental agencies regarding timely public notice of such disruptions. Noise Control: VA limits normal hours of construction to not earlier than 7:00 AM, and not later than 6 P.M. as well as limited hours of certain types of demolition and construction operations; coordinate with Contracting Officer. Muffle and shield intakes and exhausts, shroud or shield impact tools, and use electric-powered rather than diesel-powered construction equipment, as feasible, so that noise would not exceed limits per the medical center. Other than specified above in these special provisions, absolutely no construction activity shall occur during the hours of 8:00 p.m. to 7:00 a.m. which would cause noise levels to exceed the ambient noise level at building 500 without required approval from Contracting Officer. The contractor is expected to have experience working with Southern California Edison in obtaining the necessary electrical power interfaces. Contractor will be responsible for all utility connections; electrical power, potable water, sanitary sewer, steam, condensate, interior / exterior communication systems (internet, telephone, security, alarms, etc.), fire protection water, and potentially natural gas. Dust and Dirt Control: Perform the Work in a manner to minimize the generation of dust and dirt, to prevent dust and dirt from interfering with the progress of the Work, and to keep dust and dirt from accumulating in work areas and adjacent properties. Demolition of existing structures shall be performed by the contractor. Sprinkle dust causing demolition areas with water continually during demolition activity; sprinkle unpaved construction areas with water at least twice per day to reduce dust generation by approximately fifty percent (50%). Trench and exposed soil at the work site shall be watered down at least twice daily or as needed to control dust. Overfilling of haul trucks would be prevented, and protective covers shall be place on trucks if necessary to prevent dust and odor problems. Haul truck tires shall be rinsed of mud before leaving the work site and any spillage of excavated material on roadways adjacent to the work site shall be cleaned up promptly. Cover stockpiles of soil, sand and other materials; cover trucks hauling debris, soils, sand or other such material. Sweep streets surrounding demolition and construction Project site at least once per day to reduce particulate emissions. Pollution Control: Comply with applicable regulatory requirements and anti-pollution ordinances during the conduct of construction and disposal operations. Water Control: The Contractor will be responsible for the Storm Water Permit and Storm Water Pollution Plan. The contractor will not permit surface and subsurface water and other liquids to accumulate in or about the Project site and vicinity thereof. Should conditions develop, control the water or other liquids, and suitably dispose of by means of temporary pumps, piping, drainage lines, troughs, ditches, dams or other methods. Erosion Control: Plan and execute construction and earth work by methods or control surface drainage from cuts and fills, and from borrow and waste disposal areas, to prevent erosion and sedimentation. Hold the areas of bare soil exposed at one time to a minimum. Provide temporary control measures such as berms, dikes and drains. Construct fills and waste areas by selective placement to eliminate surface silts or clays, which will erode. Periodically inspect earthwork to detect any evidence of the start of erosion. Apply corrective measures as required to control erosion. Cleaning During Construction: Control accumulation of waste materials and rubbish; legally dispose of off-site at intervals approved by Contracting Officer. Clean interior areas prior to start of finish work, maintain areas free of dust and other contaminants during finishing operations. Hazardous Or Contaminated Materials The Contractor shall be responsible for the legal removal and disposal of any hazardous materials found in the Project Site. The Contractor shall review VA s Hazardous Material Reports before submitting the bid. In the event that such hazardous/contaminated material is discovered, the Contractor shall immediately notify the Contracting Officer both verbally and in writing. Upon the Government s receipt of such notification, the Contractor shall formulate and submit an abatement plan for review and approval by the VA before proceeding with this work. All Work shall comply with all applicable Federal, State and Local codes. Protection Of Existing Structures And Utilities It is the Contractor s responsibilities to evaluate soil and ground water conditions of the site and provides adequate design for all shoring, grading, and excavation. The Contractor must also locate all underground utility lines in the construction areas. The utility lines in the area must be properly relocated, maintained and properly protected from damage. Contractor shall protect existing building structures not in the Work; existing heritage trees; existing overhead, above grade and below ground utility lines, services and equipment not in the Contract, and existing walk paths, trails, roads, fencing, site fixtures, etc. from damages. Coordination: Coordinate construction activities included under various Sections of these Specifications to assure efficient and orderly installation of each part of the Work. Coordinate construction operations included under different Sections of the Specifications that are dependent upon each other for proper sequence, installation, connection, and operation. Where installation of one part of the Work is dependent on installation of other components, either before or after its own installation, schedule construction activities in the sequence required. Where availability of space is limited, coordinate installation of different components to assure maximum accessibility for required maintenance, service and repair. Make provisions to accommodate items scheduled for later installation. Locate and mark all existing utilities below grade prior to installation of work affected hereby. Prepare memoranda for distribution to each party involved outlining special procedures required for coordination. Distribute copies of these memoranda to each trade performing work at the Project site. Include such items as required notices, reports, and attendance at meetings. Prepare similar memoranda for the Contracting Officer and separate contractors where coordination of their Work is required. Administrative Procedures: Coordinate scheduling and timing of required administrative procedures with other construction activities to avoid conflicts and ensure orderly progress of the Work. Such administrative activities include, but are not limited to, the following: Preparation of schedules. Installation and removal of temporary facilities. Delivery and processing of submittals. Progress and coordination meetings. Project Close-out activities. Performance Monitoring: The Contracting Officer s Technical Representative will require a bi-weekly typewritten progress report detailing status. The Contractor will provide an independent State of California Class 1 inspector to inspect the construction site twice a week and prepare the bi-weekly report with photographs. This information will be reported to the Contracting Officer and the CFM Project Manager. Security Requirements: This project does not include anything related to patient information nor does it allow for access to the secure VA network. Government-Furnished Equipment (GFE)/Government-Furnished Information (GFI) NO computers or network access will be provided or authorized. NO Government-furnished Equipment will be issued for this project. Aside from site visits, all work will be completed at the contractor s place of work. Other Pertinent Information or Special Considerations; Inspection and Acceptance Criteria will be per the contract requirements. Risk Control; Contractor is responsible for risk control.
