Description

The Department of Veterans Affairs, Network Contracting Office 10, 24 Frank Lloyd Wright Drive, Lobby M, Suite 2200, Ann Arbor, MI 48105, intends to solicit offers for FILTER REPLACEMENT AND RECERTIFY with all labor, materials, equipment, supervision, and all necessary resources to meet the requirements below. The FILTER REPLACEMENT AND RECERTIFY shall be performed at the John D. Dingell VA Medical Center, 4646 John R Street, Detroit, MI 48201. Statement of Work 4th Floor Research Area BSC Repairs & Recertification GENERAL INFORMATION Title of Project: BSE Cabinet Repair The John D. Dingell VA Medical Center is requiring a one-time contract to replace filters and recertify the replacement filters. Scope of Work: The contractor shall provide all resources necessary to accomplish the deliverables describe in this statement of work (SOW), except as may otherwise be specified. The contractor shall provide all labor, material, instrumentation, supervision, and all other resources required to perform the following: Decontamination of 14 BSE cabinets (Information on cabinets will be below) Proper disposal of all contaminated HEPA filters. B. SPECIFIC MANDATORY TASK AND ASSOCIATED DELIVERABLES The contractor will give the Contracting Officer Representative (COR) at 8 hours noticed before starting any testing. The contractor shall communicate daily during the inspections with the COR to let him know if there are any issues with the hoods tested that day. The contractor shall notify the COR of any hoods that do not meet certification criteria and supply the COR with a quote for repairs needed to make the hood compliant prior to leaving the facility. GOVERNMENT RESPONSIBILITIES The Government will provide access to the rooms where hoods are located so the contractor can perform testing/certifications. The Government will supply room maps for all areas of work. The COR will provide an escort if needed to find rooms and locations for the contractor. G. SAFETY The contractor shall provide all personal protective equipment (PPE) their employees need such as safety glasses, hearing protection, safety shoes, gloves as required. H. REPORTS The contractor shall be responsible for submitting detailed reports within 7 days of the completion of said contract. The report is to include, the type of hood, ID number, EE number, room number, air flow data and hood pass/fail. All BSE Cabinet Testing stickers will be provided to the COR within 7 days.
