This announcement constitutes the only solicitation; quotations are being requested, and a written solicitation document will not be issued. This solicitation is a Request for Quotations (RFQ) and the solicitation number is 36C24419Q0426. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-97 effective January 24, 2018. The associated North American Industrial Classification System (NAICS) code for this procurement is 561710, EXTERMINATING AND PEST CONTROL SERVICES, with a small business size standard of $11 Million. The Department of Veterans Affairs, 642 Department of Veterans Affairs; 3900 Woodland Ave. Philadelphia, Pa. 19104 is seeking Pest Control Services. All interested companies shall provide quotations for the following: The anticipated Period of Performance will be being 4/1/2019 following Contract award and includes the following Contract Line Item Numbers (CLIN): **SEE BELOW PRICE/COST SCHEDULE Place of Performance Address: Department of Veterans Affairs Lebanon VA Medical Center 1700 South Lincoln Ave. Lebanon, PA 17042-7529 All work shall be completed in accordance with the Statement of Work/Performance Work Statement (SOW/PWS). **The SOW is included as attachment to this solicitation. Offerors are requested to quote Firm Fixed Price for any and all of the line items on the Schedule of Supplies and Services in this solicitation. Service Contract Act Wage Determination Number 2015-4231, Revision Number 07, dated 12/26/2018, applies to this acquisition and will be incorporated into the resulting contract. A copy of the Wage Determination is attached with this solicitation. Any award resulting from this solicitation will be issued on a Standard Form 1449. NOTE: To receive an award resulting from this solicitation, offerors MUST be registered in the System for Award Management (SAM) database IAW FAR 52.212-(k) & VETBIZ.
