1 SOURCES SOUGHT Project #676-19-101 STATEMENT OF WORK Repair Asphalt Surfaces Tomah VAMC, Tomah WI 54660 3/21/2019 Background: This project will address distressed pavement across the Tomah Medical Center Campus. Basically, all areas of asphalt repair will either be milling and replacing the existing pavement or sealing cracks and sealing the pavement with seal coat or slurry seal. There will be a few areas of new pavement. In all cases, attention to slope to provide proper drainage is a high priority. Magnitude of construction cost is between $500,000 and $1,000,000. Scope of Work: This construction project includes all labor, materials, equipment, tools, and supervision necessary for the construction of Project 676-19-101 Repair Asphalt Surfaces, storm inlets and sidewalks, as defined by the drawings and specifications. Major elements of this include: All road and parking lot repairs, the contractor shall provide proper drainage to storm drains or adjacent green space drainage swales. Pavement next to buildings shall be built up if needed so that rain and snow drain away from the building or structure. The expectations of work to include: Pavement milling, crack sealing, sealcoat, slurry seal, painting, new asphalt, new gravel pad, tack coat, and concrete dumpster pad. Time Requirements: Time requirement: Length of Construction 120 calendar days from signed Notice to Proceed. General Requirements: Normal working hours are 7:30AM-4:00PM. However, Contractor shall plan to work on weekends and Federal Holidays and after hours to complete this work. Work with the COR to close traffic lanes, parking lots, and reroute traffic; parking areas that are closed shall be blocked off using cones and road barriers. Provide the COR with a two-week notice prior to working in the various areas. Visits to the site by Bidders may be made only by appointment with the Medical Center Engineering Officer. Before placement and installation of work subject to tests by testing laboratory retained by Department of Veterans Affairs, the Contractor shall notify the COR in sufficient time to enable testing laboratory personnel to be present at the site in time for proper taking and testing of specimens and field inspection. Such prior notice shall not be less than three work days unless otherwise designated by the COR. All employees of general contractor and subcontractors shall comply with VA security management program and obtain permission of the VA police, be identified by project and employer, and restricted from unauthorized access. Note: NLT four (4) days after closing, responses to this Sources Sought should include a description and cost of all projects completed for this type of work, as well as percentage of labor self-performed. This Sources Sought is NOT a request for proposals or submittals of any kind. The Department of Veterans Affairs is conducting market research for potential forthcoming work and plans to issue the official solicitation on or about April 9, 2019 with the bid opening in Spring 2019. The Department of Veterans Affairs intends on processing this solicitation and all amendments via the Federal Business Opportunities website (www.fbo.gov). Vendors may search, monitor and retrieve opportunities for free; vendors may register as an Interested Vendor or select Watch Opportunity to be given notifications for any changes.
