This is a combined synopsis/solicitation for commercial supplies prepared in accordance with the format in Subpart 12.6, as supplemented with the additional information included in this notice as a set aside for service disabled veteran owned small businesses (SDVOSBs) and veteran owned small businesses (VOSBs). This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. This is a request for quotes under solicitation number: 36C26219Q0513 The Department of Veterans Affairs San Diego Healthcare System, San Diego, CA 92161 has a requirement for the replacement of eight (8) pair door low energy operators in the VA Medical Center. The Contractor shall furnish all materials/service at the Veterans Affairs, 3350 La Jolla Village Drive San Diego, CA 92161. This contract is for the replacement of 8 obsolete door operators. These operators have reached the end of their serviceable life and are no longer repairable. Contractor Will: Provide the materials and install at each door listed; One (1) Dorma ED250 pair door low energy operator. One (1) BEA LZR Safety Upgrade. Pair Door @ Kitchen Service Door, Rm. B128 Pair Door @ Linen Room, Rm. B223 Pair Door @ Corridor @ Linen Room, B225 Pair Door @ Surgery Entry North, Rm. 5044 Pair Door @ Surgery Entry South, Rm. 5063 Pair Door @ PACU, Rm. 5077 Pair Door @ PACU Hall, Rm. 5100 Pair Door @ PACU, Rm. 5091 Reuse Existing Activation Switches Reuse Existing Power Work to be done after hours to minimize disruption. Period of Performance: 6 months from the date of award. The solicitation will be in accordance with FAR Part 12, 13, and 19, Acquisition of Commercial Items, Simplified Acquisition Procedures, Small Business Programs, and Veteran Affairs Acquisition Regulations as supplemented with additional information in this notice. The North American Industrial Classification System (NAICS) code for this requirement is 238390 size standard $15 million. The Contractor shall adhere to all federal and state laws and regulations in effect during the term of this contract. The Government intends to award a firm-fixed-price contract. The Contract begins on the date of award. Due to time constraints, responses to the solicitation shall be due on Friday, March 29, 2019 at 12 Noon. Offerors are advised that it is your responsibility to review and monitor the website frequently to ensure you have the most up to date information, including amendments. The quote should be on company letterhead and provide enough information for the government to determine what the vendor is providing in material, the contractor s ability to fulfil the requirement and a cost breakdown. CONTRACTOR QUALIFICATIONS & OTHER RESPONSIBLITES: Contractor shall obtain all necessary licenses required to perform this work. Contractor shall take all precautions necessary to protect persons and property from injury or damage during the performance of this contract. Contractor shall be responsible for any injury to himself/herself, his/her employees, or others, as well as for any damage to personal or public property that occurs during the performance of this contract that is caused by him/her or his/her employees fault or negligence. Contractor shall maintain personal liability and property damage insurance prescribed by the laws of the Federal Government. Furthermore, contractor and contractor s employees performing on this contract shall have, but not limited to the following qualification, experience, licenses, and certifications: Contractor and/or contractor s employee(s) performing on this contract shall be asbestos certified because VASDHS has asbestos in the building. This asbestos requirement only applies to personnel who will do the physical installation of the equipment and peripheral parts. All other personnel that will not disturb or contact asbestos, will not be required to have an asbestos certification. Contractor shall provide the following certificates for assigned personnel prior to the performance of the contract: Current 16-hour Operation and Maintenance Asbestos Training & Current Respirator fit test. The Government reserves the right to accept or reject Contractor s employee for the rendering of services. Complaints concerning Contract Personnel s performance or conduct will be dealt with by the Contractor and COR with the final decision being made by the Contracting Officer. SAFETY REQUIREMENTS: All personnel employed by the Contractor in the performance of this contract, or any representative of the Contractor entering on VASDHS shall comply with applicable Federal, State, and VA safety and fire regulations and codes, which are in effect during the performance period of the contract. Contractor shall perform work in strict accordance with existing relevant, accredited standards and codes to minimize the possibility of injury to personnel, or damage to Government equipment. Every effort shall be made to safeguard human life and property. Contractor shall comply with all applicable Federal, State, and local requirements regarding workers health and safety. The requirements include, but are not limited to, those found in Federal and State Occupational Safety and Health Act (OSHA) statutes and regulations, such as applicable provisions of Title 29, Code of Federal Regulations (CFR), Parts 1910 and 1926, NFPA 70E, Article 130.7 29 CFR 1910.132(d)(1)(iii), 29CFR1910.137 Contractor shall provide all necessary tools, and Personal Protective Equipment (PPE) to perform the work safely, effectively, and timely. CHANGES: The awarded Contractor is advised that only the Contracting Officer, acting within the scope of the contract has the authority to make changes which affect the contract in terms of quality, quantity, price or delivery. In the event the Contractor effects any such change at the direction of any person other than the Contracting Officer, the change shall be considered to have been made without authority and no adjustment shall be made in the contract price to cover any increase in costs incurred as a result thereof. VA POLICY: Possession of weapons is prohibited. Enclosed containers, including tool kits, are subject to search. Violations of VA regulations may result in citations answerable in the United States (Federal) District Court. PARKING POLICY: It is the responsibility of Contractor personnel to park only in designated parking areas. Parking information is available from the VA Police. The VA shall not validate or make reimbursement for parking violations of the Contractor s personnel under any circumstances. IDENTIFICATION BADGES: All Contractor personnel are required to wear identification (I.D.) badges during the entire time they are on the VA Healthcare System grounds. I.D. badges must have an identification picture and shall state the name of the individual and the company represented. SMOKING POLICY: Smoking is not permitted within or around the VA Healthcare System facilities, except in designated areas. NORMAL WORKING HOURS: Contractor will shall be performing services between the hours of 7:30 AM through 4:00 PM Monday through Friday, with exceptions to National Holidays. OVERTIME AND NATIONAL HOLIDAYS: Any overtime or holiday pay that may be entitled to the Contractor s employees shall be the sole responsibility of the Contractor and shall not be billed to nor reimbursed by the Government. The Contractor is not required to provide service on the following U.S. Government holidays nor shall the Contractor be paid for these days: New Year s Day January 1 Martin Luther King s Birthday Third Monday in January President s Day Third Monday in February Memorial Day Last Monday in May Independence Day July 4 Labor Day First Monday in September Columbus Day Second Monday in October Veterans Day November 11 Thanksgiving Day Fourth Thursday in November Christmas Day December 25 NOTE: THIS NOTICE WAS NOT POSTED TO FEDBIZOPPS ON THE DATE INDICATED IN THE NOTICE ITSELF (21-MAR-2019); HOWEVER, IT DID APPEAR IN THE FEDBIZOPPS FTP FEED ON THIS DATE.
