This announcement constitutes the only solicitation; proposals are being requested, and a written solicitation document will not be issued. This solicitation is a request for quotations (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2019-01, effective 22 January 2019. The associated North American Industrial Classification System (NAICS) code for this procurement is 621999, with a small business size standard of $15.0 million. The Department of Veterans Affairs Healthcare Systems at VA North Texas Health Care System (Dallas) requires a contractor to provide Extracorporeal Shock Wave Lithotripsy (ESWL) Services with an On-Site Technician. This is a Firm Fixed-Price Contract. Period of performance shall be for one (1) Base Year with the Government having the option to exercise four (4) one-year option periods. General Timeline: i. Proposals are due Friday, 5 April 2019 at 1200 PM MST ii. Contractor performance shall begin on Wednesday, 1 May 2019. iii.
