- Date : March 22, 2019 Combined Solicitation/Synopsis Solicitation Number: 36C25219Q0455 Set Aside: Small Business Response Date: March 27,2019 NAICS Code: 311511 Dairy and Dairy Products This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) Subpart 12.6, Streamlined Procedures for Evaluation and Solicitation for Commercial Items, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being requested. This is a Request for Quote (RFQ). The government anticipates awarding a firm-fixed price contract resulting from this solicitation. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular. The North American Industrial Classification System (NAICS) code for this procurement is 311511 with a small business size standard of 1000 employees. This solicitation is for 100% set aside for Small Businesses. The NCO 12, Great Lakes Acquisition Center, 115 S. 84th Street, Suite 101, Milwaukee, WI 53214, is soliciting offers from Small Businesses to supply Jesse Brown VA Medical Center with Dairy and Dairy Products. Please provide Brand name or Equal. Respondents to this solicitation must fully demonstrate their capability by supplying detailed quote information along with any other documents necessary to support the requirements below. Quotes are to be provided to lizbeth.bradett@va.gov no later than 05:30pm CDT Wednesday, March 27, 2019. All name Brand Sellers/Resellers MUST provide an Authorized Dealer Letter.
