Description
Amendment: Service: Repair Underground Fuel Storage Tanks The contractor providing a one-time service with supervision, labor, tools, parts, and supplies to provide Underground Fuel Storage Tank Repairs of deficiencies identified during August 2018 Tri-Annual Hydrostatic Testing. The Underground Fuel Storage Tanks (UST) to be repaired are located at Bldg. 12 and Bldg. 100 and are listed in the Performance Work Statement, located on the Main Campus, of the C. W. Bill Young VA Medical Center, (CWBYVAMC) at 10000 Bay Pines Blvd, Bay Pines, Florida 33744 The following information was removed from Performance Work Statement: the Contractor shall provide the following for these personnel as a minimum, workers compensation, income tax withholding and social security payments. Building 12 Fleet The regular STP sump, regular dispenser liner, diesel STP sump and diesel dispenser liners all failed hydrostatic testing due to the presence of blue corrugated chase into the STP. Photos of the chase in each containment are included in attached PDF. (Flex OPW C15, OPW Flexworks Poly dispenser liner and sump) Building 12 (Fleet) Boot Count Regular sump = 1 piping and 2 electrical Regular dispenser = 1 piping and 2 electrical Diesel sump = 1 piping and 2 electrical Diesel dispenser = 1 piping, 2 electrical and 2 dead ends. Building 100 west side of plant Fourteen blue diversified repair boots are cracked in xerxes frp sumps and caused failing hydrostatic tests. One DW Emco-wheaton spill bucket (with EZGauge) failed a vacuum test. Hydrotest secondary to see if we need to break concrete to replace. One other DW spill bucket has water in interstice, but just passed vacuum test a few weeks ago. Pump water out of interstice. There is a copper siphon primer line that connects between sumps in an open chase pipe. We were not able to test above this pipe, it needs to be sealed and tested above this entry. A map and several photos of each sump are included in attached PDF. Building 100 boot count L1 sump = 3 piping boots, 3 electrical boots L2 sump = 3 piping boots (2 fiberglass repaired), 3 electrical boots L13 sump = 2 piping boots, 3 electrical boots, 1 siphon chase L14 sump = 3 piping boots (1 fiberglass repaired), 3 electrical boots, 1 siphon chase L15 sump = 3 piping boots, 4 electrical boots, 1 siphon chase L16 sump = 3 piping boots, 2 electrical boots, 1 siphon chase, and 1 dead end Building 100 Expansion east side of plant Fourteen of fifteen Petroleum Containment (PCI) sumps failed hydrostatic testing, because the union between the highland tank collar and PCI sump wall would not hold water. One of the spill buckets has water in the interstice, need it pumped out. (OPW Series 1-2100, 1-3100 Spill Buckets with VisiGauge). One of the manway covers was bubbling during the test and needs to be tightened down. A map and several photos are included in attachment 4.
