Description
4 This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. This solicitation 36C25019Q0464 is being issued as a Request for Quotation (RFQ), with the intent to award a Firm-Fixed Price contract for services. All solicitation documents and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-60. The contract duration is for twelve (12) months, with four (4) option years to be exercised unilaterally at the discretion of the Contracting Officer. This requirement is unrestricted-insufficient small business for commercial items under the North Atlantic Industrial Classification System (NAICS) 721110. The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, Lowest Priced Technically Acceptable factors considered in the evaluation. The following commercial service is requested: GENERAL INFORMATION 1. Title of Project: Hoptel Lodging Program for the Louis A. Stokes VAMC Cleveland, OH 44106 2. Scope of Work: The contractor shall provide all resources necessary to accomplish the deliverables described in this statement of work (SOW), except as may otherwise specify. The Louis Stokes Cleveland Department of Veterans Affairs Medical Center (VAMC), 10701 East Boulevard, Cleveland, Ohio 44106, (hereafter known as the VAMC), is seeking hotel rooms. The Cleveland VAMC will provide overnight accommodations to Veterans and a caregiver who have a scheduled appointment at the Cleveland VA Medical Center and will have to travel more than 50-miles or 2-hours from home to the appointment. 3. Background: The Cleveland VAMC has a need to contract with local hotels for overnight accommodations of patients meeting the criteria listed above, otherwise known as Hoptel Services. 4. Performance Period: The contractor shall complete the work required under this SOW in a 1-year period from the date of award, with four option periods of 1-year increments, unless otherwise directed by the Contracting Officer (CO). The Louis Stokes Cleveland Department of Veterans Affairs Medical Center (VAMC), 10701 East Boulevard, Cleveland, Ohio 44106, (hereafter known as the VAMC), is seeking hotel rooms. The VAMC estimates a need for 10 (ten) hotel rooms per night with amenities. Seven rooms (7) are guaranteed. The hotel and parking areas must be in compliance with the American with Disabilities Act. The period of performance shall be from date of award through September 30, 2023. Requirement per room is as follows: Individual sleeping/private rooms with toilet facilities for lodging of the conference attendee and other VA Medical Center (VAMC) authorized guest(s). Each room shall be commercially available industry standard furnished 3-Diamond AAA or equivalent. Room amenities include (but shall not be limited to), alarm clock, microwave, refrigerator, on site laundry facilities, two double beds or larger, private bath (sink, toilet, shower/bath tub), wake-up service, basic cable or satellite television, smoke free, and telephone. Daily services shall include, (but shall not be limited to), Daily - cleaning of room, bathroom, bed making, linen change, clean towels, washcloth and bath mat replacement, and soap replenishment, facial tissue and toilet paper replenishment. The room price must include complimentary (free) parking (i.e. standard or handicapped). The hotel must be within the following delineated area: within 10 driving miles from the VAMC as determined by MapQuest (internet) driving mileage from offeror's hotel location to VAMC. Transportation services must be provided by the hotel and available for the veteran/authorized guest to use on an as needed basis. Transportation must be available 24 (twenty-four) hours per day 7 (seven) days per week and only for transport to and from the VAMC Cleveland. 5. Type of Contract: Firm Fixed Price Indefinite Delivery Indefinite Quantity 6. Basis for Award: The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, Lowest Price Technically Acceptable (LPTA). Price and other factors considered. The following factors shall be used to evaluate offers: 1. Price 2. Transportation must be available 24 hours/7 days per week to the VAMC 3. Hotel must meet commercially available industry standard furnished 3-Diamond AAA or equivalent 4. Hotel must be within 10 driving miles from the VAMC as determined by MapQuest (internet) 5. Complimentary (free) parking (i.e. standard or handicapped). 6. Past Performance Technical and past performances, when combined, are equal to price. 7. Date, Time, and Place Offers are due: The closing date for the delivery of a response to this combined synopsis/solicitation is on or before 4:00 pm Eastern Standard Time, on Tuesday, April 8th, 2019. It is the offeror s responsibility to ensure that the transmission is received by the Government prior to the closing date and time. CONTRACT AWARD MEETING The contractor shall not commence performance on the tasks in this SOW until the CO has conducted a kick off meeting or has advised the contractor that a kick off meeting is waived. C. GENERAL REQUIREMENTS 1. For every task, the contractor shall identify in writing all necessary subtasks (if any), associated costs by task. 2. All written deliverables shall be phrased in layperson language. Statistical and other technical terminology shall not be used without providing a glossary of terms. D. SPECIFIC MANDATORY TASKS AND ASSOCIATED DELIVERABLES Description of Tasks and Associated Deliverables: The contractor shall provide the specific deliverables described below within the performance period stated in Section A.4 of this SOW. 1. VAMC will schedule conduct site visits upon receipt of proposals that meet the requirements as stated with in the solicitation. VAMC reserves the right to conduct on-site health and safety inspections during the performance period of the contract of the contractor's facilities at any time. Task One: The contractor shall provide a plan and brief for the VA project team, which presents the contractor's plan for supplying the task order. The contractor's plan shall be responsive with this SOW and describe, in further detail, the approach to be used for each aspect of the task order as defined in the technical proposal. Deliverable One: A detailed work plan and briefing. Task Two: The contractor shall provide flexible check-in and check-out procedures 24/7 to the VAMC Hotel Lodger. Upon VAMC Hotel Lodger check-in, the contractor shall request the veteran present his/her signed VAMC authorization letter which will indicate the name of the VAMC Lodger(s) and length of stay authorized. The signed VAMC authorization letter from VAMC Cleveland is the official authorization to make reservations and order rooms. A copy of this document will be faxed or emailed to the Hotel. Deliverable Two: The contractor shall verify the identity of the VAMC Lodger(s). The contractor shall maintain a Roster that each VAMC Lodger must sign daily. The Roster shall contain the date, VAMC Lodger s printed name, signature, (if applicable; VAMC authorized guest(s) printed name and signature) and room assignment.     The VAMC e-mail address or fax number to submit Roster shall be provided to the contractor at time of award. Task Three: Contractor shall email invoices to VA designated representative the 1st and 16th day of each month (only), in arrears. Deliverable Three: Each invoice shall itemize VAMC Lodgers' name and dates lodging was provided. E. EVALUATED PRICING AND LINE ITEMS Interested parties shall provide quotations for the following: 1. Minimum and Maximum Values: The guaranteed minimum daily number of rooms required under this contract will be 3 (three) rooms per night Friday, Saturday and Sunday and 7 (seven) rooms per night Monday, Tuesday, Wednesday and Thursday. The maximum rooms required by the VAMC will not exceed 5 (five) rooms per night Friday, Saturday and Sunday and 10 (ten) rooms per night Monday, Tuesday, Wednesday and Thursday. 2. Pricing and Line Items: The daily room rate offered shall include all other charges, (e.g., to include but not limited to parking, state and local taxes). Based upon VAMC requirements the contract may be modified to add additional rooms at the contract rate. The Government reserves the right to cancel at any time if lodging is no longer required at no additional cost to the Government. This contract includes options to extend the period of performance. Refer to the various option clauses for details. The contractor will be provided notice of the VAMC's intent to renew an option within 60 days of expiration of the contract. The contractor shall complete the Delivery Date column for each deliverable specified. 2. Unless otherwise specified, the number of draft copies and the number of final copies shall be the same. 3. If for any reason any deliverable cannot be delivered within the scheduled time frame, the contractor is required to explain why in writing to the CO, including a firm commitment of when the work shall be completed. This notice to the CO shall cite the reasons for the delay, and the impact on the overall project. The CO will then review the facts and issue a response, in accordance with applicable regulations. G. CHANGES TO STATEMENT OF WORK Any changes to this SOW shall be authorized and approved only through written correspondence from the CO. A copy of each change will be kept in a project folder along with all other products of the project. Costs incurred by the contractor through the actions of parties other than the CO shall be borne by the contractor. H. CONTRACTOR EXPERIENCE REQUIREMENTS The government will not dictate specific experience and education requirements of the employees initially proposed to perform the work stated herein. All Contractor employees are subject to immediate removal from performance of this contract when they are involved in a violation of the law, VA security, confidentiality requirements and/or other disciplinary reasons. I. CONFIDENTIALITY AND NONDISCLOSURE It is agreed that: 1. The preliminary and final deliverables, and all associated working papers, application source code, and other material deemed relevant by VA which have been generated by the contractor in the performance of this task order, are the exclusive property of the U.S. Government and shall be submitted to the CO at the conclusion of the task order. 2. The CO will be the sole authorized official to release, verbally or in writing, any data, draft deliverables, final deliverables, or any other written or printed materials pertaining to this task order. No information shall be released by the contractor. Any request for information relating to this task order, presented to the contractor, shall be submitted to the CO for response. 3. Press releases, marketing material, or any other printed or electronic documentation related to this project, shall not be publicized without the written approval of the CO. J. CONTRACTOR PERSONNEL SECURITY REQUIREMENTS No contractor employees shall require access to the Department of Veterans Affairs computer systems. The C&A requirements do not apply and a Security Accreditation Package is not required. The COR needs to: Ensure that a Contractor Security Control Assessment (CSCA) is completed within 30 days of contract approval and yearly on the renewal date of the contract. Ensure that the CSCA is sent to the ISO and the OCS Certification Program Office for review to ensure that appropriate security controls are being implemented in service contracts. Ensure a copy of the CSCA is maintained in the Security Management and Reporting Tool (SMART) database. COTR will provide a copy of the completed CSCA to ISO for uploading into SMART database. K. LIMIT OF VAMC LIABILITY The VAMC assumes no liability for damage caused by the veteran or his family. Any expenses incurred by the VAMC Lodger (e.g., to include but not limited to telephone use room service, internet charges, movies, room service, room butler) other than prescribed in the provisions of this contract, shall not be assumed by or the responsibility of the VAMC. The VAMC's liability under this contract is limited to the hotel room rental charge. The contractor shall explain to the VAMC Lodger and their guests that any expenses and miscellaneous charges are not the liability of the VAMC and not covered by this contract. L. FAR 52.102 states provisions and clauses should be incorporated by reference to the maximum practical extent, rather than being incorporated in full text, even if they require modification or the insertion by the Government of fill-in material or completion by the offeror or prospective contractor.   FAR 52.104(d) provides the procedure for completing fill-in information in provisions or clauses incorporated by reference. Any VAAR provision or clause may be incorporated in a quotation, solicitation or contract by reference provided the CO complies with the requirements at FAR 52.102(c)(1), (c)(2) and (c)(3).
