- ADVANCED LIFE SUPPORT (ALS) AND BASIC LIFE SUPPORT (BLS) GROUND TRANSPORTATION SERVICE This is a Sources Sought Announcement. The Department of Veterans Affairs, (VA), James A. Haley VA Hospital located in Tampa, FL. is conducting market research to identify contract holders that are qualified Service-Disabled Veteran-Owned Small Business (SDVOSB) and Veteran-Owned Small Business (VOSB), capable and interested in providing the required services as listed in the attached Performance Work Statement document. The Government may use the responses to this market research for information and planning purposes. The responses to this announcement will assist in market research survey to determine the availability of SDVOSBs and VOSBs in consideration of a socio-economic set-aside. The attached description document describes only the currently contemplated possible scope of the services and may vary from the work scope in a final Performance Work Statement document if the Government decides to issue a Request for Quote. Interested Vendors The applicable NAICS for this service is: 621910. To be considered for this requirement, vendors must be registered in the System for Award Management (SAM - https://www.sam.gov) and be identified as a licensed service provider for ambulance service on the Hillsborough County Public Transportation Commission website (http://www.hillsboroughcounty.org/index.aspx?nid=2595). If a vendor is not currently identified as a licensed service provider, they must be able to successfully register prior to submitting any offer associated with this requirement. Interested vendors/contractors submitting a response to this inquiry must include the following information: 1. Company Information (business name, DUNS number, business size/category, and any pertinent company-related information). 2. Primary point of contact, including phone number and e-mail address 3. Applicable socioeconomic categories and any pertinent information, which demonstrates firm s ability to meet the requirement 4. Capability Statement (brief descriptions of capabilities, resources, experience, etc. 5. GSA or other Government contracts schedule numbers, if applicable.
