Description

This is a solicitation notice 36C24419Q0466; Pittsburgh VAMC, Health Care, is seeking a vendor that can provide the Simbionix LapMENTOR virtual reality trainer. The issue date of the solicitation will be Mar. 27, 2019. This requirement will be brand name only, the manufacturer is 3D Systems, part number LP10100. If quoting as a dealer or distributor who can provide the requirement above. The vendors responding must be certified by 3D Systems to deliver, uninstall present equipment, and install the new trainers. (Trade In discount may be required). The trainers are to be purchased for the VA Pittsburgh Healthcare where the system will be installed in BLDG 29, 2nd floor, room 2M312. All vendors must be registered in SAM. The dealers and distributors must provide certification from 3D Systems that they will be allowed to purchase and install 3D equipment All Vendors interested in, and with the capability and capacity to fulfill this requirement, are required to furnish the following minimum information in reply to this solicitation: Company Name, address, Government Business Point of Contact, telephone number and email address. Company DUNS number, CAGE Code, and Tax ID numbers. Identify any current Federal Supply Schedule, GSA Contracts held, if none then state that no FSS/GSA schedules are held. Responses shall be submitted electronically, via email to Donna Toliver at donna.toliver3@va.gov due no later Wednesday 04/03/2019 at 1:00 pm.
