Description

Scope of Work Project Title: Renovate Oncology Pharmacy Project Number: 630-17-104 Contractor shall provide all labor, materials, equipment, transportation and supervision necessary to renovate Oncology Pharmacy, upgrade selected offices and corridor areas as indicated in the construction documents. All work shall be performed at the Veterans Affairs Medical Center located on 423 East, 23rd Street, New York, NY 10010. Scope of Work: Complete renovation of existing Oncology Pharmacy on the 12th Floor, conversion of existing offices into a new Oncology Pharmacy, and all Main Corridors and Elevator Lobbies as indicated in the Construction Documents. Provide materials handling equipment, project management, supervision and coordination to complete one (1) Oncology Pharmacy cleanroom modular including equipment per attached Terra Universal s Quote #207495 and Specification Section 13-21-13. Contractor/Terra Universal shall perform leak testing for all HEPA filters during the testing and certification of the clean rooms. All demolished materials shall be removed off the site on a regular basis and during off hours (5:00 PM 11:30 PM). All disposal and rubbish removals are the contractor s responsibility. Hold Bi-Weekly construction meetings and prepare meeting minutes. Provide 12 independent third party monthly construction safety inspections and reports. Coordinate all fire alarm, sprinkler system and utility shutdowns with facility representative and attend utility shutdown meetings as requested by the Resident Engineer. Coordinate materials delivery and storage with facility representative. Provide Interim Life Safety Measures (ILSM) to mitigate construction safety deficiencies. Provide Infection Disease Control and Risk Assessment (ICRA). Provide an independent testing laboratory to perform water sampling and testing and written reports in accordance to VA plumbing specifications and Water System Infection Control Risk Assessment (WSICRA) requirements. The independent testing laboratory must be certified by the New York State Environmental Laboratory Approval Program (ELAP). Contractor shall be required to provide the following documentation for all of his/her personnel working on VA property: VA issued identification badges OHSA Training certifications Construction Safety and Accident Prevention plans TB tuberculin test certifications Base Bid and Alternates: Base Bid: Include Items 1 through 11 listed above Completion Time: 730 calendar days (2 Years) Deduct Alternate #1: All work included in Base Bid items except delete Phases 5 and 6 from Scope of Work in the Construction Documents Completion Time: 540 calendar days (1.5 Years) Deduct Alternate #2: All work included in Base Bid items except delete Phases 3, 4, 5 and 6 from Scope of Work in the Construction Documents Completion Time: 365 calendar days (1 Year) NOTE: THIS NOTICE WAS NOT POSTED TO FEDBIZOPPS ON THE DATE INDICATED IN THE NOTICE ITSELF (01-APR-2019); HOWEVER, IT DID APPEAR IN THE FEDBIZOPPS FTP FEED ON THIS DATE.
