- NOTICE: The U.S. Department of Veterans Affairs, VA Northern California Health Care System has a requirement for a Primary Care Community Based Outpatient Clinic (CBOC) in Yreka, California. The VA anticipates soliciting for an Indefinite Delivery, Indefinite Quantity, Capitated contract on or around April 3, 2019. This requirement is being solicited as a small business set-aside. However, it is noted that the solicitation will contain VAAR 852.215-70, Service-Disabled Veteran-Owned and Veteran-Owned Small Business Evaluation Factors (JUL 2016) (DEVIATION). The NAICS Code for this requirement is 621498 All Other Outpatient Care Centers and the small business size standard is $20.5 Million. In accordance with FAR Clause 52.204-7; Contractors are required to be registered with the System for Award Management (SAM) when submitting an offer or quotation and shall continue to be registered until time of award, during performance and through final payment of any contract resulting from this solicitation. DESCRIPTION: Primary Care CBOC is defined as one that: offer both medical (physically on site) and behavioral health care (either physically on site or by telehealth) and may offer support services such as pharmacy, laboratory, and x-ray. Primary Care CBOCs are required to provide both primary care and behavioral health services. Sites that do not provide both primary care and behavioral health services are classified as Other Outpatient Services. Access to specialty care is not provided on site but may be available through referral or telehealth. A Primary care CBOC often provides home-based primary care (HBPC) and home telehealth to the population it serves to meet the primary care and behavioral health needs of Veterans who have difficulty accessing clinic-based care. Primary care in VA includes both medical and behavioral health care services, as they are inseparable in providing personalized, proactive, patient-centered health care. Primary Care Requirements. Services shall be provided in a private hospital, office or clinic environment to veterans, primarily residing in Siskiyou County and the surrounding area of Northern California. It is estimated that Request for Proposal (RFP) 36C26119R0001 will be available for download from this website on or about April 3, 2019. No hardcopies will be sent.
